-statement that the resident may file a complaint with the director concerning resident abuse,
neglect, misappropriation of resident property
-the most recently known addresses and telephone #s for the dept, office of secretary of family
and social services, ombudsman, area on aging, local mental health center, adult protective
services

Display of facility information - ANS -prominently displayed
-provided to residents and applicants for admission oral and written information
-how to apply for and use Medicare / Medicaid benefits
-how to receive refunds for previous payments covered by such benefits

Facility must inform each resident of - ANS -name, specialty, and way of contacting the
physician responsible for his or her care

Protection of Resident Funds - ANS -resident has the right to manage his financial affairs
-facility may not require residents to deposit their personal funds with the facility
-only with written authorization of the resident the facility must hold and manage and account for
personal funds of the resident deposited at the facility

Depositing Residents personal Funds - ANS -facility must deposit any of the personal
funds if more then $50 in an interest bearing account that is separate from the facility's
operating account and credits all interest earned on the patients funds to his or her account

, Facility must: - ANS -provide reasonable access during normal business hours to the funds
of the account
-return to the resident in no later than 15 days upon written request
-provide reasonable access during normal business hours
-the individual financial record must be provided to the resident or his legal rep upon request
and through quarterly statements

Grievances- Resident has the right to the following: - ANS -voice a grievance without
discrimination
-prompt efforts by facility to resolve grievances
-recommend changes in policy / procedure and receive reasonable responses to their requests
without fear of reprisal
-each facility should have policies for investigating and responding to complaints and grievances

Access and Visitation Rights - ANS facility should provide reasonable visiting hours, at
least 9 hours a day

Transfer and Discharge rights - ANS -HFs must permit each resident to remain in the
facility and not transfer or discharge the resident from the facility, unless reason

Reasons allowed for transfer / discharge - ANS -its necessary for the resident's welfare
and needs cannot be met at the facility
-appropriate because the resident's health has improved sufficiently
-safety of the resident is endangered
-resident has failed to pay after reasonable notice
-the facility ceases to operate

Nursing Services - ANS -must have sufficient nursing staff to provide nursing and related
services to attain or maintain the highest practicable physical, mental, and psychosocial
wellbeing of each resident, as determined by the resident assessment and plans of care

Nursing services facilities must provide: - ANS -provide a licensed nurse hour-to-resident
ratio of .5 licensed nurse hour per resident per day
-must designate a licensed nurse to serve as a charge nurse on each tour of duty
-must use the services of a registered nurse for at least 8 consecutive hours a day, 7 days a
week
(can be waived if needed)

Nursing Aides - ANS -facilities can not use individual as nurses aid for more than 4 months
unless person has completed training and competency evaluation program approved by the
state
-registry verification, multi-state verification
-education annual for all nursing aides (12 hours)
-if haven't worked in 24 months as CNA, must complete new course
