NURS 251 Exam 2
CN 1 - ANS Olfactory
CN2 - ANS Optic
CN3 - ANS Oculomotor
CN4 - ANS Trochlear
CN5 - ANS Trigeminal
CN6 - ANS Abducens
CN7 - ANS Facial
CN8 - ANS Acoustic
CN9 - ANS Glossopharyngeal
CN10 - ANS Vagus
CN11 - ANS Spinal Accessory
CN12 - ANS Hypoglossal
Name the 3 sensory CNs
OOA - ANS -Olfactory
-Optic
-Acoustic
Name the 5 motor CNs
OTASH - ANS -Oculomotor
-Trochlear
-Abducens
-Spinal Accessory
-Hypoglossal
Name the 4 mixed CNs
TFGV - ANS -Trigeminal
-Facial
,-Glossopharyngeal
-Vagus
Which CNs come off of the midbrain? - ANS -olfactory
-oculomotor
-optic
-trochlear
Which CNs come off of the pons - ANS -trigeminal
-abducens
-facial
-acoustic
Which CNs come off of the medulla - ANS -glossopharyngeal
-vagus
-spinal accessory
-hypoglossal
Name the CNs that can be clumped together to check - ANS -3,4,6 (oculomotor, trochlear,
abducens)
-5,7 (Trigeminal, facial)
-9,10,12 (glossopharyngeal, vagus, hypoglossal)
How would you test the olfactory CN 1 - ANS Give pt a sample of something to smell
(something common) and ask them what they believe it is.
This nerve smells impulses from nasal mucous membrane and sends the sense to the brain -
ANS olfactory
this cranial nerve sends visual impulses from eye to brain - ANS optic
What tests can you use to test CN2, optic - ANS -visual acuity (Snellen)
-Visual Fields (check optic discs)
Explain the Snellen test and what does it test for - ANS - the snellen test checks teh optic nerve
- you must have teh pt stand back 20 ft away and ask them to read the lowest line they can and
see how far they cam read it to.
What does it mean when a pt has a 20/40 vision - ANS he can see at 20ft what people can see
at 40ft
this cranial nerve controls eye muscle contraction, eye movement, constricts pupils, and
elevates eyelids - ANS the oculomotor nerve
,How can you test oculomotor, trochlear, and abducens and explain the test
6EEP - ANS -the six cardinal fields of gaze
-Extraocular eye movement (can the eye get there)
-Eye muscle movement (lazy eye)
-Pupillary response
What is a lazy eye and when is it checked - ANS its checked when checking for CNs 3,4,6
-it occurs when the muscles surrounding the eyes are too strong, you can be born with it or it
can be because of stress on the eyes from reading and such.
Explain when checking CN 3,4,6 what pupillary responses you're checkign for - ANS
-consensual responses
-accomodation
what happens when checkign CN3,4,6, for pupillary accomodation - ANS close items will
constrict the pupils
items at a distance will dilate the pupils
What happens when checking CN 3,4,6 for pupillary consensual reaction - ANS you shine light
in the left pupil to constrict it and then the automatic response is for the right pupil to constrict as
well.
When checking the CN 3,4,6 how are you supposed to describe your pt's results - ANS PERLA
What is PERLA - ANS A way to size up the pupils and look at a shape
it stands for
Pupils equal round react to light accomodation.
this cranial nerve controls contracts eye muscle and eye movement - ANS trochlear
this cranial nerve controls lateral eye movement - ANS abducens
Explain how you would assess the sensory and motor portion of CN 5 and 7 - ANS Sensory:
use a cotton ball to rub pt's face on the three region (sharp and dull object) also have pt identify
sweet, sour, and salty objects
Motor: Have pt clench teetch, smile frow
n, wrinkle forehead, etc
this cranial nerve controls hearing and balance - ANS acoustic
How do you test CN 8, acoustic - ANS whisper in their ear on both sides have one side blocked
This cranial nerve controls taste, gag, salivary glands - ANS Glossopharyngeal
, This cranial nerve controls sensation from the throat, larynx, heart, lungs, bronchi, GI Tract,
swallowing, talking, digestion. - ANS Vagus
This cranial nerve controls neck muscles, shoulder, head rotation, and larynx - ANS Spinal
accessory
This cranial nerve controls tongue, food movement, and talking. - ANS hypoglossal
how would you test CN 9,10,12 - ANS -Have pt open mouth and say "Ah"
-Stick tongue out (notice if its midline, tremors, or deviations)
-Have pt say "night, flight, dynamite"
How would you test CN 11, Spinal accessory - ANS -have hands on pt shoulders and have the
pt shrug
-have pt push their head against the palm of your hand.
What are the 5 parts of an interview
GETBD - ANS 1.) Gather complete and accurate data about the person's health state, including
description, chronology, and any symptoms of illness
2.)Establish trust so that the person feels accepted and thus free to share all relevant data
3.) Teach the person about his or her health state
4.) Build rapport for a theraputic relationship which facilitates future future diagnosis, planning,
and treatment
5.) Discuss health promotion and disease intervention
What are the 3 phases of the interview IWS - ANS Introductory phase
Working phase
summary and closing phase
During this phase of interviewing one introduces themselves, they explain the purpose of
interview by discussing questions, explain reason for note taking, and assure confidentiality, also
you are building a rapport during this phase - ANS introductory phase
during the introductory phase what is occurs - ANS -introduce oneself
-explain your purpose of the interview
-discuss questions
-explain reason fro note taking
-assure confidentiality and privacy
-time frame of interview
-build a rapport
How should you address a patient - ANS by using his or her surname not by their first name
