HPI (History of Present Illness): Ally Chen, a 44-year-old Caucasian female, experiences
significant seasonal mood fluctuations that impact her productivity and mood as daylight
decreases. Over several years, she has observed a pattern where depressive symptoms emerge in
winter, contrasting sharply with her energetic, engaged state during warmer months. This
cyclical pattern affects her role as a high school biology teacher, where she struggles to meet her
usual standards, participates less in school activities, and reduces interactions with students. Her
symptoms include profound fatigue, concentration difficulties, a lack of motivation, social
withdrawal, and disrupted sleep, often waking multiple times at night. She also reports increased
carbohydrate cravings and weight gain during winter, worsening her self-esteem and depressive
symptoms. However, she experiences no psychosis, and thoughts of hopelessness are occasional
