SITUATION
Narrate your care by explaining that you are going to give report. Invite the patient to participate.

BACKGROUND
Off-going nurse provides admitting diagnosis, brief history, status, primary/consulting providers,
allergies, precautions, diet, plan of care, discharge plans, etc.
ASSESSMENT
Review V/S including pain, clinical status. Use words the patient will understand
Check IV’s, equipment, incisions, dressings, drains, foley, tubes, ostomy, skin, etc.
Use the teach-back method for medication side effects/purpose or safety/plan of care
Both nurses scan for safe environment (bed alarms, fall risk, room free of clutter) and move items
within reach (table, call bell/TV remote, phone, water and garbage can)
RECOMMENDATIONS
Review plan of care with patient
Oncoming nurse updates and reviews information on the Patient Communication Board
CLOSING AND THANK YOU
Off-going nurse thanks the patient for the opportunity to provide care and manages up the oncoming
nurse
Let the patient know when they can expect to see you or another team member next
Both nurses wash or foam hands
