What is the definition of HF?

✓ a complex clinical syndrome that can result from any structural or

functional cardiac disorder that impairs the ability of the left ventricle

to eject blood or fill with blood."

What is an ejection problem in HF?

✓ rEF (there are situations which the myocardium covers the aortic

output so you have cardiac muscle or other skin covering that

opening, so this is not technically rEF and is often treated through

surgery or slowing down the HR)

What is the problem with pEF?

✓ a)Pumped in atrium instead of aorta (MVR)

b)Obstruction of opening of aorta

i.IHSS (idiopathic hypertrophic subaortic stenosis)

ii.HOCM (Hypertrophic Obstructive Cardiomyopathy)

iii.Aortic Stenosis

others

How is blood pumped in atrium instead of aorta?

✓ left atrium contracts pushing into LV, stretches and goes out through

aorta (when valve malfunctions between atria and ventricle, mitral

valve regurge). When ventricle contracts, it doesn't all go out to aorta,

it goes back into atria too; fix with surgery

What causes lack of coordination of LV?

✓ nerves around LV that simultaneously at rapid speeds cause the LV to

contract at the same time. If a nerve is not conducting correctly, you

don't have good coordination (can cause bundle branch blocks or

other issues with nerves going around the ventricles).

How is blood normally pumped?
