Overview of the respiratory system. Two tracts and functions.
Upper respiratory tract warms and filters air.
Lower respiratory tract (Lungs) is where gas exchange occurs.
Both tracts work in conjunction with each other to deliver oxygen to the body and expel
carbon dioxide.
Recognition of increased work of breathing (WOB) (6)
* Stridor
* Use of accessory muscles (intercostals, sternocleidomastoid, scalene)
* Expiratory wheezes with other key findings
* Intercostal retractions
* Inability to speak in whole sentences
* Cessation of expiratory wheeze without improvement in condition
Diagnostic evaluation and tests for respiratory dysfunction (11)
* Pulmonary function tests (PFT's)
* Arterial blood gases
* Venous blood gas studies
* Imaging studies (Chest x ray, CT scan)
* Pulse ox
* End tidal Carbon dioxide
* Sputum studies
, * Cultures
* Endoscopic Procedures (Bronchoscopy)
* Thoracentesis (Procedure that removes air or fluid from thoracic cavity for therapeutic
relief or diagnostic reasons)
* Biopsy of the lung (Removing tissue from the lung to screen it for cancer)
Atelectasis
Atelectasis is airway inflammation and edema that lead to the collapse of the alveoli at
the terminal ends of the airway where gas exchange occurs
* Can be an acute or chronic process
* Obstructive or non-obstructive
* Can occur with certain inflammatory and infections such as pneumonia, COPD, Covid
19, and sometimes TB.
Atelectasis Symptoms (5)
* Can be insidious, increasing SOB and WOB, cough and sputum production
* Acute- tachycardia, tachypnea, pleural pain, central cyanosis if areas of lungs are
affected
* Chronic- same as acute but pulmonary infection may be present
* Crackles to decreased breath sounds
* Hypoxemia if SPO2 is less than 90%
Atelectasis Prevention (7)
*The ultimate goal for prevention is to improve ventilation and remove secretions
* Frequent turning
