1. History of Present Illness

, Objective: To elicit detailed information so as to characterize the testicular pain,

establish the sequence of events that led to this presentation, and identify

associated symptoms that may guide possible diagnoses.




Onset and Duration: Ascertain the time of onset of pain and the nature of its onset-

sudden or gradual. Sudden onset is very characteristic of testicular torsion, while

gradual onset is more typical of epididymitis.

Location and character of pain: Ask the patient whether the pain is unilateral or

bilateral. Unilateral pain is often seen in torsion and epididymitis, while a more

generalized ache may be indicative of other causes.




Severity and Progression: Ask the severity of pain to be scored by the patient on an

intensity scale of 1-10 and whether it has been getting worse or staying the same.

Torsion is consistent with a rapidly increasing, high-severity pain.

Associated Symptoms: Fever, nausea, vomiting, abdominal pain, or voiding

problems. Nausea and Vomiting may accompany torsion, and fever may signify an

infection such as epididymitis.
