De Theory Of Constraints (TOC) is continu bezig met verbeteren, maar gericht op een bottleneck. Om
een probleem op te lossen moet je je richten op de bottleneck, daar ontstaat het probleem dus daar
zit ook de oplossing. De TOC werkt vanuit het principe dat de output van een systeem bepaald wordt
door de zwakste schakel.
ToC werkt aan de hand van ketenaansturing; (drum-buffer-rope ; DBR-concept, om bottlenecks in de
procesflow aan te passen): de middelste bepaald het tempo, de afdelingen erna zien hoe de
middelste het doet en kunnen rekening met hem houden en er ook niet voorbij. De afdelingen voor
de middelste die zitten vast met een touw en worden teruggetrokken wanneer de middelste afdeling
het niet bij houd. Er is wel een buffer tussen de constraint en de volgende afdeling voor wanneer er
deze bijvoorbeeld een keer net iets meer produceert.
Het doel van ToC is output verhoging, de focus ligt dus minder op de kwaliteit van zorg en op de
patiënt.
De TOC bestaat uit 5 stappen:
1. Identificeer – vind de bottleneck of beperkende factor;
2. Exploiteer – benut de capaciteit van de bottleneck maximaal;
3. Subordinate (pas andere activiteiten aan) – maak de rest van het systeem ondergeschikt;
4. Elevate – hef de bottleneck op;
5. Herhaal – keer terug naar de eerste stap;

- Identificeer: je weet hoelang/hoeveel iets kost om te maken, je weet ook wat de capaciteit is
op jouw afdelingen. Zoek uit waar de capaciteit het minst toereikend is, dit is je bottleneck.
- Exploiteer: bedenk maatregelen om de capaciteit optimaal te benutten (denk aan betere
machines of iets dergelijks)
- Subordinate: je kijkt naar het proces en past deze aan, aan de constraint. Wat genereert bv
de meeste throughput bij de constraint, daar begin je met produceren zo werk je door.
- Elevate: je implementeert een verandering waardoor de capaciteit groter wordt. Hierdoor
hef je de bottleneck op.
- Herhaal: er is nu een andere bottleneck zoek deze op en doorloop het proces opnieuw.
Elke organisatie heeft tenminste 1 beperking die in de weg staat van het behalen van het algemene
doel. ToC veronderstelt systeemdenken, dit uit zich in het kijken naar de volledige logistieke keten, in
plaats van het focussen op een schakel binnen de keten.
