best understood as the result of a process in which coders

values - answer coders select individual letters and number, called _____ in a

specific aspect of the procedure and can have up to 34 different values, consisting of
the ten digits (0 - 9) and 24 letters (A - H, J - N, and P - Z). Not every character uses all
34 values. For some sections and some characters, only a few values are used while in
others, most of the values are used. Each Section of the manual designates how each
character of the code is used within that Section. The definition of each value, for
example 1 or A, is based on the character it occupies. Examples will be provided.

Medical and Surgical. - answer The options and values for a character vary based on
the first character of the code, which identifies the Section. The largest Section is

code is found.

Character 2 defines - answer the Body System in which the procedure is performed.

Character 3 defines - answer the Root Operation, or the objective of the procedure.

Character 4 defines - answer the Body Part, or specific anatomical site where the
physician performed the procedure.

the procedure site.

Character 7 defines - answer a Qualifier for the code, which describes additional
information about the procedure.

Character 1: Section - answer Character 1: Section

, The largest Section is Medical and Surgical. Other Sections in ICD-10-PCS classify
medical and surgical-related procedures, such as Obstetrics or Chiropractic, and
ancillary services, such a Nuclear Medicine or Mental Health.

Character 2: Body System - answer The second character in the Medical and
Surgical Section identifies the Body System, the general physiological system or
anatomical region involved, such as central nervous system or endocrine system. The
Medical and Surgical System has 31 possible values for the Body System. The Body
System values do not always correlate to the commonly defined organ systems.

For example, the Skeletal System has five (5) Body System values, each referring to a
specific component.

Body System Values for the Skeletal System


Because within each PCS Body System there can be up to 34 values for Body Parts,
codes can be more specific when large systems are divided into multiple values. This
enables the skeletal system, for example, to have up to 170 Body Part values. If the
Skeletal System were defined with only one (1) value, then the number of Body Part
values would be limited to 34.

Many Sections other than Medical and Surgical also use Character 2 to describe Body
System, but may also use it for other purposes. The Placement Section (Section value
2) uses Character 2 for Anatomical Orifice. The Physical Rehabilitation and Diagnostic
Audiology Section (Section value F) uses Character 2 for Section Qualifier, which is a
broad type of service such as Rehabilitation.


NOTE: The Body System values in PCS are not related to the letters used in ICD-10-
CM diagnosis codes. For example, codes for the gastrointestinal system begin with the
letter K. In PCS coding, the gastrointestinal system has a Body System value of D.

Character 3: Root Operation - answer he third character in the Medical and Surgical
Section, and most other Sections, defines the Root Operation, which describes the
objective of the procedure (excision, destruction, extraction). The options and values for
Character 3 vary from one Section to the next and from one Body Part value to the next
within a Section. Root Operations are Main Terms in the Index, so coders must be
familiar with their names and numbers. The names and definitions of Character 3
appear at the top of each table in the ICD-10-PCS manual. In addition, an appendix in
most ICD-10-PCS manuals defines all Root Operations for the Medical and Surgical
Section, provide expanded examples, and gives examples.

For example, a Latin-based Medical term such as gastrectomy can mean removing all
or part of the stomach. The definitions of the words excision and resection are
sometimes used interchangeably in common medical usage, such as stomach excision
