CONSIDERATIONS
Indications
-Ensure intervention does not go against Dr’s orders, for example if
order indicates complete bed rest
-repositioning/movement is needed to promote circulation, healing, Nursing Interventions (pre, intra, post)
Pre: Assess- pt’s mobility, how much pt can do, pt’s weight, if
ROM and to prevent pressure ulcers assistance is needed considering weight, considering injuries and/or
weak sides, pt’s willingness, pt’s LOC, assess what supplies/devices
will be needed ex: hoyer lift, transfer board or other assistive
devices, prepare environment & remove obstacles
Intra: use good body mechanics, keep body alignment, distribute
weight among large muscles, use assistive devices, follow facility’s
policy for safe client handling
Post- ensure pt is seated in a safe position without risk of falling or
losing balance, ensure pt comfort
Outcomes/Evaluation
-prevention of skin breakdown due to pressure on sitting/laying

area for prolonged time -Explain procedure to pt before beginning, tell pt how they can
