Community Health Promotion among the Elderly with Type 2 Diabetes
Living in Hamilton, Ontario
Type 2 diabetes is becoming a leading public health problem, with a significantly

growing trend in rapid social-economic developments and lifestyle changes in the last

decade. As of 2017, 7.2% of Canadians aged 13 and above (approximately 2.2 million

citizens) reported being diagnosed with diabetes. The proportion of males diagnosed

with diabetes increased from 7.5% in 2016 to 8.5% in 2017 while the ratio of females

remained consistent between the two years (Statistics Canada, 2018). The City of

Hamilton in the province of Ontario is one of the regions hardest hit by Diabetes type 2.

This article examines the prevalence, the cost and the control of diabetes type 2 in

older people. The study aims to comprehend the burden and the control of diabetes

type 2 among the older people, and secondly, to understand the health policy program

adopted and the role of the nurse in responding to the disease.

Community Health Concepts/Nodes

The primary function of the community health nurse is to treat patients. Besides,

community health nurse educates the community about maintaining their health so that

they may minimize chances of contracting the disease and even mortality. The

community health nurse plans the place of meeting, handout, undertakes screening

process on patients, distribute medication, and supervise the immunization process.

The research done by Nikitara (2019 ) shows that nurses are the basis of any

triumphant social healthcare plan and instrumental in executing any private or

government-financed health plan. Community health nurses should use improved

verdict, making plans like the nursing procedures, which coalesces decision, actions,

accountability, and responsibility. Community health nurses should take time to update
