Introduction

According to our required textbook, “Error is defined as the failure of a planned action to

be completed as intended or the use of a wrong plan to achieve an aim.” (Johnson & Sollecito,

2018). Human error affects all elements of healthcare, not just one component. It is imperative to

ensure that healthcare practitioners take these errors into account when devising strategies to

guarantee patient safety. Human errors can be avoided by being aware of them and knowing

what needs to be done to prevent them from happening again. In order to help enterprises

identify all the risk factors that could pose a danger to the organization and how to mitigate them,

SEIPS was developed. In order to improve patient safety and care, it's critical in the healthcare

industry to know how to lower errors and avoid potential hazards.

Seven Types of Errors

, Every healthcare company has errors from time to time. Institution and management

mistakes, skill-based, rule-based, knowledge-based, choice errors, perceptual errors, and human

errors are the seven categories of errors that can occur in a healthcare institution. When

management doesn't accept responsibility for any problems or events that arise, organizational

blunders ensue. For instance, when a manager enters patient data incorrectly and then disputes

that they did so. If extra care is taken when entering patient data, such as double-checking the

information, errors like this one can be avoided.

Errors based on skill are defined as actions taken as a result of unintentional activities. An

illustration would be if a nurse prepared to insert a drug picked up the wrong syringe and gave
