Nephrotic SyndromeDefinition: Immunologic kidney disorder in which glomerular permeability increases so larger
molecules pass through the membrane into the urine and are then excreted.
Pathophysiology: The process causes massive loss of protein into the urine, edema formation
and decreased plasma albumin levels.
Causes/ Risk factors: The most common cause of glomerular membrane changes is altered
immunity with inflammation. Defects in glomerular filtration can also occur as a result of
genetic defects of the glomerular filtering system such as Fabry disease.
Symptoms:
Massive proteinuria
Hypoalbuminemia
Facial edema
Lipiduria
Hyperlipidemia
Delayed clotting
Reduced kidney function with elevated BUN and serum creatinine
Decreased GFR
Diagnosis: Severe proteinuria- more than 3.5g protein in a 24 hour urine sample.
Treatment:
ACE inhibitors- helps to decrease protein loss in urine and can improve blood lipid levels.
Heparin- may reduce vascular defects and improve kidney function.
Diet changes
Mild diuretics- may be needed to control edema
