Nephrotic Syndrome

Definition: Immunologic kidney disorder in which glomerular permeability increases so larger
molecules pass through the membrane into the urine and are then excreted.

Pathophysiology: The process causes massive loss of protein into the urine, edema formation
and decreased plasma albumin levels.

Causes/ Risk factors: The most common cause of glomerular membrane changes is altered
immunity with inflammation. Defects in glomerular filtration can also occur as a result of
genetic defects of the glomerular filtering system such as Fabry disease.

Symptoms:
 Massive proteinuria
 Hypoalbuminemia
 Facial edema
 Lipiduria
 Hyperlipidemia
 Delayed clotting
 Reduced kidney function with elevated BUN and serum creatinine
 Decreased GFR

Diagnosis: Severe proteinuria- more than 3.5g protein in a 24 hour urine sample.

Treatment:
 ACE inhibitors- helps to decrease protein loss in urine and can improve blood lipid levels.
 Heparin- may reduce vascular defects and improve kidney function.
 Diet changes
 Mild diuretics- may be needed to control edema
