Quality Performance and Management and Methods

, A1 Initiatives

Fielder Medical Center in New York has Quality Improvement initiatives in place to

measure (how are they doing), assess (are they meeting expectations) and improve (how they can

improve performance) patient care. One of them is Central Line-Associated Blood Stream

Infections (CLABSI). Central line catheters are inserted into the vein of neck, chest or groin to

give medications or fluids or to collect blood. They can remain in place for weeks or even

months and therefore can cause infections such as bloodstream infection. Bloodstream infection

happens when bacteria or viruses enter the blood thru the central line. To reduce the rate of

infections, Fielder Medical Center created a team to research ways on the best cost-effective

practice and came up with the program called “Stop the Line”. They trained staff on how to

properly insert central line catheters and how to continue maintenance while the patient has the

line in place. The results after 18 months were astonishing as infections dropped nearly 50%

bringing their rates below national average

Cather-Associated Urinary Tract Infections (CAUTI) is a urinary tract infection caused

by a urinary catheter in which bacteria or viruses travel along the catheter and into the bladder or

kidney They can be long term such as when one is recovering from surgery or unable to move

easily. An infection, if not treated properly can turn into unpleasant symptoms, excessive use of

antibiotics or even sepsis, the body’s response to an infection that can shut down body organs.

Proper care and maintenance of urinary catheters is extremely important to reduce the rates of

CAUTI.therefore, Fielder Medical Center organized a team of healthcare providers and

caregivers to identify and implement the best way to prevent infections throughout the hospital.

They worked with other hospitals to gather information on ways to prevent this issue. The team

trained the staff on other uses besides urinary catheters and educating nurse and physicians on
