Introduction
Performing a comprehensive health assessment allows nurses and healthcare
professionals to get an overall insight into a patient’s health status by addressing several
domains. The Social Determinants of Health (SDOH) are amongst the various domains assessed
and provide an understanding of how social, economic, political, legal, and material factors
affect health and disease in individuals (Global Health, 2012). Social determinants include a
wide variety of assessments but are narrowed down into five categories: economic stability,
education, social and community context, health and health care, and neighborhood and built
environment. Healthy People 2020 (2020) brings attention to the importance of addressing these
social determinants in people by making it their goal to “create social and physical environments
that promote good health for all”. Having an understanding of a patient’s social determinants
facilitates a stronger foundation for the nurse-patient relationship and improves health while
reducing healthcare disparities.
Interview Techniques
There are many fundamental interview techniques available for completing a
comprehensive health assessment and assessing social determinants. The interview techniques
that I chose to utilize when assessing Carolyn Cross’s social determinants were active listening,
guided questioning, adaptability, and summarization. By implementing these techniques
throughout the interview, a respectful and comforting environment was established which
created a sense of trust and safety for Ms. Cross throughout the discussion. It is beneficial to the
plan of care to form a trustworthy nurse-patient relationship from the beginning.

SOCIAL DETERMINANTS OF HEALTH
Active listening is a key factor and effective technique when assessing social
determinants (Bickley, Szilagyi, & Hoffman, 2017). When a patient is speaking, the provider
must be actively listening by connecting with the patient emotionally without causing any
interruptions. This technique allows the patient to feel heard and gives them comfort in sharing
their concerns. By actively listening, the provider provides the patient with a sense of
understanding and security.
The use of guided questioning is another interview technique used to elicit more
information from the patient while allowing the provider to facilitate full communication
(Bickley, Szilagyi, & Hoffman, 2017). The patient continues to use their own words and share
their thoughts and concerns, all while the provider sustains interest and encourages hearing the
full story. Without using open-ended and guided questions, the patient can feel restricted by yes-
or-no answers and leave out important details.
Partnering is an additional method that was used when discussing Carolyn Cross’s social
determinants of health. It is important to build rapport with the patient during the discussion. By
partnering with Carolyn Cross, she is made to feel supported. Demonstrating a commitment to
the patient and developing an ongoing relationship allows the patient to feel that they will
continuously be cared for by the provider regardless of what happens (Bickley, Szilagyi, &
Hoffman, 2017).
Lastly, using a summarization technique with Carolyn Cross proved to be very
purposeful. As the provider, demonstrating understanding, getting all the details correct, and
establishing what you know and do not know ensures that the entirety of the patient’s
information is discussed and understood. This is done by reviewing what information they have
shared, allowing the patient to explain more, and also gives the provider a chance to convey their
