Patient History
Analysis

Summarize the patient history, explaining key patient demographics and

family history that could be risk factors for common diseases.

Patient has a history of gastritis due the extreme use of alcohol intake. She also has

a history of hypokalemia. The patient also had a fractured wrist most likely due to use of

alcohol. Patient has had mild angina and was prescribed Lanoxin and Lasix for what could

be for arteriosclerotic cardiovascular problems. This information is not verified. The patient

has a family history of alcoholism on mother’s side and cardiovascular disease on father’s

side. Her brother had a heart attack related to coronary artery disease. With her family

history alcoholism and cardiovascular diseases can be likely in this patient. She denies

heavy drinking but is a moderate smoker.

Identify the past diagnosis (or diagnoses, if more than one exists in the file) and explain

how the diagnosis was made. Specifically, what tests were done?

, The patient has four diagnoses listed in her patient history. Arteriosclerotic

cardiovascular disease, alcoholism, hypokalemia, and gastritis. The focus is on the current

diagnosis of arteriosclerotic cardiovascular disease, but there is mention of the past history

that could also

help with diagnosis and treatment of current condition.

To properly diagnose Arteriosclerotic cardiovascular disease a doctor may use a CT

scan or MRA to study the patient’s arteries. The imaging can often show hardening or

narrowing of large arteries, or abnormalities in the artery walls. There are other methods

doctors can use to diagnose this disease such as cardiac catheterization angiogram, stress

test, ECG, ankle-brachial index, doppler ultrasound, or blood tests [ CITATION May18 \l

1033 ].

Alcoholism diagnosis is a bit trickier. For a doctor to properly diagnose they will

ask several questions about drinking habits, along with maintaining permission to speak to

family and friends about the patients drinking habits. The doctor will perform a physical

examination and there are many physical signs that indicate complications of alcohol use.

Lab tests and imaging can show certain patterns and abnormalities that point to abuse of

alcohol, the imaging can sometimes show damage to your organs [ CITATION May181 \l

1033 ].

Hypokalemia can be diagnosed from Serum potassium measurement, ECG, and when

the mechanism not evident clinically, 24-hour urinary potassium excretion and serum

magnesium concentration [ CITATION Jam20 \l 1033 ].

Gastritis most likely from alcohol use can be diagnosed through either a physical

exam of the stomach and abdomen or an internal exam using a tool placed in the esophagus.

A breath test may be done, to find a certain bacterium [ CITATION Isa19 \l 1033 ].
