B. Werkingsmechanisme
Werken in op fase 0 van het actiepotentiaal: ze vertragen de snelle depolarisatie
o Werkt vertragend op hartritme
Gecombineerde effect: Door het vertraagde hartritme is er ook een verminderde
calcium instroom in de hartspier.
o Cardiodepressie: negatief chronotroop, dromotroop en inotroop
Membraan stabilisatoren worden ze om bovenstaande redenen ook genoemd

Aromatisch deel (met of zonder substiuenten):
Lipofiele interacties aan de bindingsplaats het
natrium kanaal.
Brugfunctie (ester, amide, ether, keton,

Alkyl-keten (C1-C4): lipofiele en sterische
interacties met de interactie op het ion-kanaal.

B. Werkingsmechanisme
Functie funny soidum channel: Een speciaal natriumkanaal dat verantwoordelijk is
voor de geleidelijke depolarisatie in de sinusknoop tijdens fase 4 van het
actiepotentaal. Dit kanaal heeft een tragere natrium instroom. Het is cAMP
afhankelijk.
‘Funny sodium channel’ inhibitor: gaat de diastolische depolarisatie vertragen en zo
ook het hartritme.
o Het hart heeft meer tijd om zich te vullen met bloed, dus meer bloed en
zuurstof aanvoer in het hart.
Niet negatief inotroop: Hier is er geen sprake van minder calcium instroom. Enkel de
diastolische depolarisatie zal vertragen.

Specifiek: Angina pectoris
Algemeen Na-Kanaal blokker: lokaal anestheticum, antiaritmica, anti-apileptica
