Compact hoisting machinery: Machinery wirh a gross vehicle weight not exceeding 10,000 lbs, excluding Class 1, Class 3 and Class 4 hoisting machinery as listed in 520CMR 6.10. Injury and incedent documentation: A form furnished by the department detailing all specific info regarding any serious injury, property damage or mechanical failure that may have occurred 22. inspector: A District Engineering Inspector employed by the Department 23. Manufacturer approved attachment: Attachable equipment tailored to perform a particular task, designed or approved for use in accordance with the equipment 5 / 23 manufacturers spec's. Operating procedures for hoisting machinery: In addition: 1) a Written and signed record of all inspections shall be kept with the hoisting machinery and made available at the site for examination by the department that conforms the regulations are 29 CFR 1926.1412 2) prior to starting any machinery operator must complete walk around of the equipment to verify people are clear of equip and that its safe condition 3) Operator shall not engage in any practice that will divert their attention while 9 / 23 engaged in operating the hoisting machinery 4) At no time shall the operator of machinery operate or be required to operate the machinery beyond it's rated capacity without the manufactures written approval which shall include a new chart showing the new limitations. other than assembly/disassembly the use of the load moment indicator override key ,or entering a false crane set up criteria to increase rated capacity, is strictly forbidden 5) operator shall operate hoisting machinery only one fully attentive the operator shall not operate the equipment erratically and/or under the influence of alcohol or drugs when operators physically or mentally unfit the operation shall not operate with the machinery
