Otitis Media

One nursing diagnosis that could be allocated to this patient would be disturbed sensory

perception related to age related ocular-changes as evidence by the patient wearing bifocal lenses, using a

magnifying glass to see, reporting not being able to see at night, and patient’s physician recommended the

patient see an eye specialist. The patient’s short-term goal will be that the patient is able to verbalize an

understanding of the ocular-changes she is facing and how to continue her ADLs with these changes

within three days. One long term goal for this patient would be patient will take the necessary steps to

meet with an eye specialist and decided if surgery is an option to regain her visual field somewhere in

the near future. The first nursing intervention for this patient would be provide adequate lighting to

make sure patient can carry out ADLs to eliminate risk of injury. Rational being that the elderly needs

more adequate lighting than younger people (Senior Vision: Over 60 Years of Age, n.d.).

The second nursing intervention for this patient will be encouraging the patient sees their

ophthalmologist for six month or yearly check-ups and provide information in regard to the visual

diagnosis she has. Rationale being this allows a person who specializes in this patient’s particular issues

so that her progress can be monitored, and care can be given to enhance her visual acuity (Senior Vision:

Over 60 Years of Age, n.d.). The second nursing diagnosis for this patient could be risk for falls related to

ocular changes due to aging. The patient short-term goal would be to remain free of falls for six months.

The patient’s long-term goal is to incorporate strategies into their ADL that will increase their safety and

prevent falls for months. The first nursing intervention will be educated the patient on the advantages of

wearing the appropriate eyeglass, hearing aids, and assistive walking devices. Rationale being eyeglass,

hearing aids, and walking devices help eliminate hazardous situations because they promote visual and
