Directions: Use this template to develop professional speaker notes for the in-
person Practicum Project: Health Promotion Collaboration meeting with licensed
healthcare professionals. Your meeting should be approximately 15-30 minutes.
Complete the following sections using information from Weeks 1-3 Practicum
Project discussions and assignment.
Introduction of Self and Practicum Project
In one paragraph introduce yourself: include your name and credentials;
your affiliation with Chamberlain University, the program of study, course;
and the purpose of the Health Promotion Collaboration meeting.
My name is Marissa and I have been a nurse for about a year and a half. Previous to that
I worked as an EMT in the ER for 6 years. I am currently in my last semester at Chamberlain in
the RN to BSN program. I will graduate in November! The purpose of this meeting is to speak
with you about how we can collaborate and create new educational resources.
Introduction of Topic
In one or more paragraphs, identify your selected Health Promotion topic,
why you selected it, and why it is a community health concern.
My selected topic is substance use/abuse in my community. Substance use/abuse effects
all kinds of people. No matter the age, financial, or educational status. Believe it or not, Cape
1
, Cod has a problem with substance use. We see a lot of patients on our unit with cardiac issues
related to substance abuse. This is a significant concern in our community.
Professional organizations view substance use/abuse as a community health concern
because it affects everyone. It is shown that, "Monitoring trends in SUDs gives public health
providers opportunities to assess the scope of the issue in the United States and to plan substance
abuse prevention and treatment programming" (Lipari & Van Horn, 2017, p. 1). My goal is to
bring more awareness to this issue so we can work together to find a solution to help educate and
help our community.
Demographic Data
In one or more paragraphs, compare the county, state, and national
demographic data. Discuss how data are similar and how they are different.
The demographic data acquired for Barnstable County, Massachusetts, and the United States
are similar in many ways but also different. In Barnstable County, Massachusetts, and the U.S.
have similar percentages for males/females in the population. They also have similar percentages
for people who have a disability under the age of 65, all being within 8%.
However, there are many differences between the county, state, and national data. Starting
with the average mortgage cost, the county and state share similar numbers of about $2,500. The
national average is about $1,800. That is almost half of what people are paying in this area! You
can also see this difference in the average home cost. Houses in Barnstable County cost just
under $500,000, same as Massachusetts. The national average is, again, almost half that at
$280,000. The average household living in Barnstable County makes about $120,000 a year. In
