NR 451 ASSIGNMENT WEEK 6 EBP CHANGE PROCESS FORM
NR 451 ASSIGNMENT WEEK 6 EBP CHANGE PROCESS FORM Star Point 1: Discovery (Identify topic and practice issue) Identify the topic and the nursing practice issue related to this topic. (This MUST involve a nursing practice issue.) Surgical infections are one of the critical issues in healthcare facilities. The infection has various adverse effects that include the infection of new illnesses, the deterioration of the existing illness and even death. The topic of evaluation is the rising incidences of surgical infections in healthcare facilities to establish the possible intervention measures. Briefly describe your rationale for your topic selection. Include the scope of the issue/problem. Surgical infections have led to the reduction in patient safety thereby making hospitals inefficient in discharging their role of providing efficient and effective patient care. The problem, which is unavoidable on the patient’s side, has led to the development of various complications after surgery as well as death of millions of patients across the world. It is therefore imperative to define the problem and its causes, which would help develop possible intervention measures to reduce or eliminate incidences of surgical infections. Star Point 2: Summary (Evidence to support need for a change) Describe the practice problem in your own words and formulate your PICOT question. The high rate of surgical infections caused by errors and negligence continues the practice problem. The infections range from failure to use Chlorhexidine as outlined in pre-operative procedures, lack of due care and professionalism, common errors and negligence by health care provides. Despite the wide-ranging effects of surgical infections, there is lacks sufficient intervention measures to avert the situation. List the systematic review chosen from the Cochrane Database of Systematic Reviews from the Chamberlain library. Type the complete APA reference for the systematic review selected. Anderson, D. J., Podgorny, K., Berrios-Torres, S. I., Bratzler, D. W., Dellinger, E. P., Greene, L., ... & Kaye, K. S. (2014). Strategies to prevent surgical site infections in acute care hospitals: 2014 update. Infection Control & Hospital Epidemiology, 35(S2), S66-S88. List and briefly describe other sources used for data and information. List any other optional scholarly source used as a supplement to the systematic review in APA format. 1. Cataife, G., Weinberg, D. A., Wong, H. H., & Kahn, K. L. (2014). The effect of Surgical Care Improvement Project (SCIP) compliance on surgical site infections (SSI). Medical care, 52, S66-S73. 2. Cataife, G., Weinberg, D. A., Wong, H. H., & Kahn, K. L. (2014). The effect of Surgical Care Improvement Project (SCIP) compliance on surgical site infections (SSI). Medical care, 52, S66-S73. 3. Semsarzadeh, N. N., Tadisina, K. K., Maddox, J., Chopra, K., & Singh, D. P. (2015). Closed incision negative-pressure therapy is associated with decreased surgical-site infections: a meta-analysis. Plastic and reconstructive surgery, 136(3), 592-602. Briefly summarize the main findings (in your own words) from the systematic review and the strength of the evidence. Patient safety is one of the key components and aim of healthcare providers. It is therefore imperative to come up with the relevant measures to maximize patient safety and minimize errors that may put the patient at risk. The rise is surgical infections is mainly due to avoidable human and operational errors. The implementation of measures to reduce the prevalence is a vital step in improving healthcare safety and efficiency. Outline one or two evidence-based solutions you will consider for the trial project. The use of Chlorhexidine as part of the pre-operative procedure is critical in reducing and preventing surgical errors. Chlorhexidine helps reduce incidences of breakdown and irritation, which puts patients at a higher risk of infection. The use of Chlorhexidine requires prior examination to establish whether there is change in strength or regimen specifically among patients with sensitive skin. Another solution is the avoidance of hair removal at surgical sites. Hair removal increases the chances of having cuts and irritations that exposes patients to infections. The alternative to hair removal is the use of clippers if possible. Star Point 3: Translation (Action Plan) Identify care standards, practice guidelines, or protocols that may be in place to support your intervention planning (These may come from your organization or from the other sources listed in your Summary section in Star Point 2). Centers for Disease Control SSI Prevention Guideline Category IA recommendations provides the relevant d=guidelines and operational procedures that healthcare providers should follow to avoid or reduce instances of surgical infections. List your stakeholders (by title and not names; include yourself) and describe their roles and responsibilities in the change process (no more than 5). 1. Nurses and Physicians Surgeons are directly involved in the operational practice. Following of the prescribed procedures and measures by surgeons would greatly reduce instances of surgical infections. 2. The Management The management develops policies and strategies that promote patient safety and culture within the healthcare facility. They also provide training to its staff on the operational procedures and guidelines and supervise them in their roles. 3. Patient Patients are involved in the operation procedure and should give the medical staff all the necessary cooperation and information of any existing allergies and infections. What specifically is your nursing role in the change process? Other nursing roles? Nurses are directly involved in assisting the physician in the surgery procedure. They should therefore be keen to note any unprocedural activities. In addition, they should report to the necessary authorities any dangerous and unsafe practices. List your stakeholders by position titles (charge nurse, pharmacist, etc.).-Why are the members chosen (stakeholders) important to your project? Supervisors-5 – the supervisors are responsible for ensuring that the relevant procedures are followed in a surgical procedure and that staff have the necessary equipment to conduct an operation. Physicians- 3– Physicians should ensure that they exercise due care and professionalism when undertaking surgery. What type of cost analysis will be needed prior to a trial? Who needs to be involved with this? The primary cost is in training. About 10 physicians and 8 nurses will be trained in the trial period. The process will also entail an awareness campaign that will involve printing of checklists, flashcards, notices and video displays. Star Point 4: (Implementation) Describe the process for gaining permission to plan and begin a trial. Is there a specific group, committee, or nurse leaders involved? The nurse leaders and head physician jointly writes to the hospital management to seek permission while outlining the problem, causes, impacts and the need for change. The intervention measures to be implemented in the change process will also be outlined in the letter. Describe the plan for educating the staff about the change process trial and how they will be impacted or asked to participate. The parties involved will be involved in training on the problem, the need to make changes and the recommended intervention measures. Communication on the viability of the changes will be key to ensure everybody is on-board. The training process will be practical and convincing to ensure that all the involved parties acquire the relevant knowled Outline the implementation timeline for the change process (start time/end time, what steps are to occur along the timeline). 01.05.2019 to 14.02.2019 Training of staff on the need to change 15.05.2019- 28.02.2019 Development of advertisement materials 01.06.2018- 30.03.2019 Development of cognizance on the policies and strategies to reduce or eliminate surgical infections and uphold patient safety 01.07.2018- 31.04.2019 Evaluation of trained concepts and skills List the measurable outcomes based on the PICOT. How will these be measured? What forms, if any, might be used for recording purposes during the pilot change process. Describe. What resources are available to staff (include yourself) during the change pilot? Will there be meetings of certain stakeholders throughout the trial? If so, who and when will they meet? Star Point 5: (Evaluation) How will you report the outcomes of the trial? What would be the next steps for the use of the change process information?
