There are many diseases and conditions, different forms of them and multiple diagnosis
and prognosis. In unfortunate situations when a patient is told about their disease or
condition, many questions are formed. Their thoughts mainly consist of negative
outcomes alongside with stress which can lead to overthinking, this will increase
concerns. There are three main possible outcomes of all conditions, these are: life time
management, complete recovery and eventual death. These outcomes may change
from patient to patient and can vary on the disease/condition. When many hear that the
condition has a 98% chance of recovery, they may not take it as serious. However there
is always the small majority that is part of the 2% that couldn't make it. Being mindful but
also keeping a positive mindset alongside with the support of professionals ensures

Care pathways consist of forming a secure pathway of care for the patient, also
ensuring that the pathway is flexible incase of any sudden events. This pathway will set
a comfort zone for the patient, they will be educated which emphasises support. One
main step of forming a care pathway is presenting an initial assessment of needs. This
step will consist of the patient meeting with a professional/ specialist of the disorder.
This would be an important step as the patient will acknowledge the information more
carefully as the profession will be outlining the main resources available, treatment
options and addressing concerns. Care pathways consist of answering questions and
showing the patient support.

are available for personal research. Care pathways will have clear treatment options
available for the patient, emotional support will also be provided. Having diagnosed with
any type of condition can affect all people and friends emotionally, throughout and after
the treatment constant check ups and if needed mentoring will be necessary. Many
concerns may build up throughout the process of getting scans, tests and confirming the
condition, doctors must ensure that all concerns are addressed. Patients must always
feel confident and knowledgeable on all steps, they should never feel as if they haven't
received enough support or information. There are many types of care settings that the
patient will be placed in and referred to within their care pathway, these consist of 4
main types which are: primary care setting, secondary care setting, Tertiary care setting
and Domiciliary care setting. Care pathways are necessary and present for every type
of diagnosis, however they are slightly more important and common with cancer.
Pathways consist of the confirmation of the diagnoses, multiple scans etc depending on
the condition and slowly the treatment being followed up if there is a treatment. By the
end of the care pathway, the main aim is for the recovery however if there isn't a chance
of recovery then the care pathway ensures treatments upon treatments are formed for
the patients quality of life. As mentioned above, there are three main outcomes: life time
management, complete recovery and eventual death. Another purpose is also making
the patient feel better physically and mentally. Initial assessments of needs will be
consistent throughout the care pathway, available resources, treatment options,
address concerns will be answered throughout sessions and appointments with
professionals eg. specialist of disorder. There also may be many settings involved
throughout the care pathway, such as
