Requirements for Taking an Acknowledgement - ANSWER - Ensures signer appears before them with at least one form of ID which provides a physical description of signer (ie Drivers licenses). Not needed if notary personally knows signer or there is a witness to also identify the signer
Ensures that the document is filled in completely. Not a legal review but instead for completeness. If there are blanks signer must place an X or strike through the blank
Ensures that the signer understands the title and is signing willingly and freely. Notary may ask basic questions of the substance of the document to ensure these are correct.
Must be signed, dated and stamped (Stamp must include the the expiration date of commission. If they do not have a stamp, the must write his or hers commission expiration date)

What must a Journal Entry Include? - ANSWER - 1. Date and time of Notary Act
2. Type of Act (ie an Acknowledgement)
3. Title of Document
4. Date Document was signed
5. Signature (printed name and address of each signer and if applicable each witness)
6. Form of ID (journals should be bound to prevent tampering)
Proof of Execution - ANSWER - When a signer can not be present infant of the notary, therefore a Witness who knows the signer must be present for when the signer is signing the document and also sign the document as the witness. Then the witness must take an Oath or Affirmation infront of the notary declaring they watched the signer sign the document. It is advised that this only be done for Deeds.
What is a Jurat - ANSWER - Certificate of office or person before whom writing was sworn and is designed to compel truthfulness on part of the signer. Completed during an Affidavit. Must sign before the Notary the signer must take an oath or affirmation
