Hoorcollege 1 Introductie bacteriologieHeel veel bacteriën en veel verschillende soorten. Verschilt per
plek in lichaam welke soorten.
Microbioom
- Voeding en metabolisme van voeding
o Afbraak van polysachariden
o Productie vitamine B en K → kunnen wij zelf niet maken
- Maturatie en instructie van immune systeem
- Kolonisatie resistentie → bescherming tegen indringers
Risico is dat er ook pathogenen tussen zitten, die af en toe ziekte kunnen
veroorzaken.0020
Commensalisme → micro-organismen die zich vermenigvuldigen maar geen ziekte
veroorzaken
Infectie → aanwezigheid (invasie) van micro-organismen die zich delen in de host,
en daarmee schade aanbrengen aan de host. Hebt ook opportunistische
pathogenen die aanwezig zijn in gezond microbioom
- Neisseria meningitides, streptococcus pneumoniae, staphylococcus aureus,
group A streptococcus
Pathogeen = micro-organisme dat ziekte veroorzaakt
Pathogeniteit = vermogen om ziekte te veroorzaken in de host
Virulence = de hoeveelheid pathogeniteit van microbe
Virulence factor = microbial component die bijdraagt aan disease-making ability
Carrier = host die potentiële pathogeen bij zich draagt zonder ziekte
Kan verschillende klinische beelden hebben die veroorzaakt worden door zelfde
pathogeen → S. aureus kan Scalded Skin syndrome en voedselvergiftiging
veroorzaken.
Komt door verschillende route van contaminatie en ook de eigenschappen van de
bacterie. Maar ook door de eigenschappen van de host.

- Huid, speeksel membranen (Wond infecties, STD)→ SSSS (Staphylococcal
scalded skin syndrome)
- Transcutaan (via vectoren, insecten)
- Bloedbaan (IV drug gebruik, tand verwijderen)

- Gastrointestinaal (bedorven voedsel) → voedselvergiftiging
Invasiveness → mogelijkheid om weefsel binnen te dringen; kolonisatie en host
defence vermijden
Toxigenesis → mogelijkheid om toxins te produceren; exotoxins (uitgescheiden) en
endotoxins (zitten aan baxcterie vast)
Virulence factors → adhesives, invasins, toxins, capsule, enzymes, pili
Staph aureus
- Scalded skin syndrome
o Exfoliative toxins A en B
o Toxins komen in bloedbaan terecht
o Beschadeging van desmosomen
- Voedselvergiftiging
o Enterotoxin A
o Gaan tussen de cellen door
- toxins
o cytolytic toxins → tissue afbraak, abces formatie
o enterotoxins (A-E) → toxiciteit
o exfoliative toxin → blistering; verlies van desmosomen
- enzymes
o Coagulase → omzetting firbonogen tot fibrine
o Hyalurodinase, lipase fibrinolysis → verdeling van tissues
o Catalase → bescherming tegen zuurstof radicalen
Eigenschappen van de host
- Kind, volwassenen, ouderen
- Gezond of verminderde resistentie
- Vaccinatie
Diagnostiek → inflammatoire cellen en gram
positieve cellen
Drie soorten van staphylococcus → S. aureus, S.
epidermis, S. saprophyticus. Zijn commensaal
maar opportunistisch.
Hebt ook klinische beelden die op elkaar lijken maar veroorzaakt worden door
verschillende bacteriën. Voorbeeld is bacteriële meningitis dat kan worden
veroorzaakt door S. pneumoniae of door N. meningitidis
Van N. meningitidis word gevaccineerd tegen type A, C, W en Y. niet tegen B.

mogelijk multipliceren en mogelijk schade en
mogelijke ziekte van de host.

Aantonen bacteriële etiologie
- Aantonen van agens → microscopie, kweek, nucleïnezuur detectie, antigeen
detectie
- Aantonen immunoglobulinen tegen agens = serologie (indirect)
Meeste bacteriën hebben een celwand, kan dik zijn of
dun zijn, afhankelijk van hoeveelheid peptidoglycaan
lagen. Dikke laag → gram positief (paars). Dunne laag →
gram negatief (roze).
Zuurvaste staven → tuberculose.
Diplococcus → twee coccus
Streptococcus → ketens
Staphylococcus → clusters
Media; vast (Koch), vloeibaar (Pasteur)
- Selectief/ niet-selectief (bloedplaat → veel bacteriën kunnen groeien)
- Incubatie; milieu (bepaalt wat wel en niet kan groeien), duur
- Steriel/ niet-steriel
Pneumonie, sputum → s. pneumoniae, H. inlfluenzia, S. aureus
UWI, urine → E. coli en andere coliforme staven, enterokokken
Wondinfectie, pus of wonduitstrijk → S. aureus, GAS

- Biochemisch/enzymatisch
- MALDI-TOF MS
- Sequentie analyse
Resistentie bepaling
- Verdunningsreeks maken
- Inoculatie met bacterie
- Incubatie
- Kijk naar de groei → MIC (minimal inhibitory concentration) =
laagste concentratie waar geen groei is
Kan ook kijken naar genoom met PCR en kijken of daar resistentie genen zijn.
Gebeurt niet vaak. (nucleïnezuur detectie)
Nucleïnezuur detectie → ook gebruikt voor bacteriën die moeilijk/niet te kweken zijn
