Androgens: uses, effects, side effects and monitoring
Androgens (Androgel 1%, Axiron, Testim, Testoderm, Androderm, Striant)
Testosterone is the primary male androgen
Used for deficiency associated with hypogonadism in males, and in both sexes for cancer and
HIV, treats libido, endometriosis, and postmenopausal symptoms. Illicit use to enhance athletic
performance and increase muscle mass.
Patient education
Men who use Androgel should avoid skin to skin contact with pregnant women
Cholestatic hepatitis and jaundice may occur with low dose androgens
Digital prostate exam needed before and during therapy
Long term use requires monitoring of Ca, testosterone, PSA, liver function,
glucose, and lipids
ADR – MEN- gynecomastia, reduced sperm levels acne, baldness depression, headache,
paradoxically decreased libido, scrotal/groin pain, sustained erection, urination changes.

WOMEN- mental irregularities, hypercalcemia
Prolonged use associated with hepatitis, hepatic neoplasms, cholestatic hepatitis, jaundice, and
hepatocellular CA, GI symptoms, clotting factor suppression, increased RBC production
Antiandrogens: Antiandrogens (Finasterde, Dutasteride, Avodart, Lupron) - androgen inhibitors
(aka 5-alpha reductase inhibitors) used for BPH, male pattern baldness, prostate CA,
endometriosis, uterine fibroids
Direct antiandrogens: inhibit androgen uptake.
Flutamide – prostate CA
Sprionolactone – K+ sparing diuretic (off label: female hirtuism, acne)
Estrogens uses, effects, contraindications, side effects, monitoring
Estrogens- (Premarin, Estradiol) replacement therapy, used in combination therapy for
contraception use, in menopause will treat hot flashes, vaginal atrophy, and irregular periods.
Contraception pills used for non-contraceptive use includes dysmenorrhea, cycle control, acne,
hirsutism due to polycystic ovary disease.
Effects of estrogen include cycle control and maintenance of normal structure of
the skin and blood vessels
ADR – M.I., HTN, alternation in clotting factors, if used in women with an intact uterus will
cause abnormal bleeding, endrometrial hyperplasia, and gyne cancers.
Contraindications of estrogen therapy include history of any type of cancer, women with
intact uterus, women who have migraines with aura which can increase stroke risk.
