Meeting care and support needs.

Introduction: health care professionals did not work together in the past this meant that
assessing their services was difficult and problematic. This resulted in people's individual care
being by the different people they needed to see within their care plan. It was not due to services
being unavailable but because of the poor communication between professionals. A team that
consists of a variety of professionals working alongside each other and communicating in order
to provide care for an individual is called a multidisciplinary team, the professionals can be
working in the same sector for example health care or from different sectors like education and
health care. This type of team will arrange to meet on a regular basis whether its weekly or
monthly it depends on the individuals needs in which they are caring for and how much support
or arrangements will need to take place. Multi-disciplinary teams would generally be split into two
teams; essential people who are the core of the care plan and could also have an extended team
which includes professionals who have much less involvement with the case. Nevertheless, both
teams will provide holistic care and support of the needs of the patient, by working in partnership
and in collaboration. This will overall meet the recommendation and quality of care or recovery
for the patient.

Patricks case…
GENERAL PRACTITIONER: The multi-disciplinary team supporting Patrick would involve a
counsellor, nutritionist and general practitioner. This is stated in the case study when the GP
involves the nutritionist and counsellor in supporting him with the treatment plan that he and
Patrick created together. Patricks life required necessary changes; these are stated in his
treatment plan. A multidisciplinary team always has a key worker and it can be any member of the
team but in this case the GP has this role as they were the first point of contact within the team.
The GP has the responsibility in creating a working relationship with Patrick, and to administer a
person-centred care plan. It is also the GP's role and responsibility to make Patrick a referral to an
appropriate service, reason being Patrick's GP will have a greater understanding about his
treatment and health history more than any other professional. his medical records are held at
the GP Practice, and therefore, the GP will base the decisions for a referral on this knowledge. A
General practitioner is required in Patrick's Multi-disciplinary team and they will be truly
beneficial. This is because the GP was Patrick’s first point of contact the main benefit of the go
being the key worker of the team is because of their location they will be available at Patrick’s
local practice not too far from his home therefore if he has worries or wants to change something
he feels is wrong, he can do it easily. For example, if he found that his joint pains were worsening
he can easily book an emergency appointment on that day, with the GP, instead going to a walk in
centre having to wait for a long time in pain to essentially receive the same advice, diagnosis or
even be rejected and told to see a GP due to having more emergency cases which need to be seen
before Patrick.
CHANCELLOR: As Patricks chancellor it is not their role to provide Patrick with advice but to
encourage and help him to make his own decisions. By actively listening to Patrick it will support
the birth of a clear relationship consisting of trust and respect that will influence Patrick to feel
open and comfortable with the counsellor, this will result in a more efficient care plan and benefit
Patrick the most. Respecting Patrick's confidentiality and privacy is a basic requirement for
maintaining trust and respecting his life choices. Patrick must be able to talk about problems he
normally would not discuss with other people with his chancellor. As he knows he isn’t going to be
discriminated against because of having HIV. The chancellor will be friendly and non-judgmental
and therefore present a willingness to support him in this time. Patrick suffers from anxiety, low
self-esteem and also feels as if he is an unnecessary weight on other people’s lives. Patrick can
improve on his mental health and self-concept with his chancellor. His chancellor can provide
advice and encouragement to help him become more confident. By being more confident it will
allow him to interact with others without feeling intimidated or criticised.
