and choice.

Explain the benefits of personalisation in the context of supporting selected individuals.
For instance, suggesting that Nusrat should take a lead role at the youth centre to
improve her confidence and self-esteem.

Person-centred care is a way of thinking and doing things that sees the people using health and social
services as equal partners in planning, developing and monitoring care to make sure it meets their needs.
This means putting people and their families at the centre of decisions and seeing them as experts, working
alongside professionals to get the best outcome. Personalisation is quite similar as it is all about recognising
the importance of individuality and it encourages service users to have more control and choice over their
treatment. Carr (2008) stated that personalisation focuses on placing the individual at the centre of their care
and an understanding that they know best what their needs are and how to meet them. As an example,
Nusrat would be provided a Makaton grid since she has difficulties communicating with others due to her
learning difficulties, by doing so she is abler to input her thoughts and opinions about her care because she
knows her own needs best.

Supporting each individual by ensuring they receive the care and support needed to meet their individual
goals is vital in a health and social care setting. It allows all service users to have more control and choice
over their care, therefore promoting individuality and empowerment. Individual care also helps people to
identify their own needs as well as their own beliefs about how they should be treated.

 For Nusrat, her care plan may include support with relationships through a therapist. As she has learning
difficulties she may find it hard to from relationships but she can get help to try and form meaningful
relationships at her local theatre group.
 For Brenda, her care plan may include access of the use of facilities/ services in her local community for
example mental health groups to help her cope with any changes to her health within the sheltered
accommodation.
