Assess how multi-agency and multi-disciplinary teams work together to provide co-
ordinated support – what are the benefits?
E.g. and Autistic child may have involvement with the following agencies and
professionals:
NHS – (GP, Paediatrician, clinical psychologist, counsellor, speech and language
therapist)
Local Authority and education services (social worker, SENCO, educational
psychologist)

By definition a multidisciplinary team is a group of health and social care professionals who have different
roles and work together in order support an individual or family when facing a complex situation such as the
diagnosis of a disease or illness. Multidisciplinary teams are essential within a health and social care setting
because it means that service users are guaranteed treatment with professionals who understand and have
expert knowledge of each area such as an oncologist for the prevention and treatment for cancer or
haematologist for diseases associated with blood. But in certain cases, these professionals will work as part
of a team to deliver treatment for example a patient with leukaemia would have a multidisciplinary team
including an oncologist and a haematologist because leukaemia is a type of blood cancer.
Multidisciplinary teams are most successful in health and social care because they take a holistic approach
towards care, treatment and support, this means looking at all the person’s needs and their abilities and
discussing ways they can be met. As an example, Nusrat may have a multidisciplinary team consisting of a
GP, nurse and a neurologist. Each professional has a different role within Nusrat’s support and care.
