In this report I will explain the benefits of promoting personalisation when overcoming challenges
faced by individuals with different needs. I will be focusing on two case studies which are of Billy and
Valerie. I will explain personalisation and the different plans.


Care Study 1: Valerie B. aged 24 years

Valerie B. was admitted to the community hospital, following surgery for her appendix which burst
before she could reach hospital.

Valerie has mental health problems which began when her parents died when she was six years old.
Valerie’s surgery was complicated due to her being obese. She has low self-esteem and cannot see
why she needs to lose weight or change her lifestyle.

Valerie lives in flat provided by a housing association, but has been bullied by local teenagers because
of her obesity.

Valerie’s surgical wound is healing, but she does not want to go home because she is afraid that the
bullying will start again. She is unemployed and receives Universal benefit.

Valerie has a hearing impairment but has never been assessed for any support to improve her
hearing. Valerie is a vegetarian.

Case study 4: Billy G. aged 18 years

Billy is homeless and was admitted to the community hospital by a passing paramedic, when he had
an asthma attack in the street. The city hospital did not have beds and so a place was found in the
community hospital.

Billy left home following an argument with his mother’s partner, which resulted in a violent fight
between Billy and the man. He has been living on the streets since this incident. The weather is cold
and damp which contributed to the asthma attack.

Billy has a visual impairment and finds street living difficult. He cannot clearly read road signs or find
his way around the city easily. Billy has complained about stomach pains since being admitted to
hospital, which could be due to him living mainly on food from rubbish bins, thrown out by local
restaurants.

Billy cannot go back home from hospital, because his mother has changed the locks and has refused
to visit him.

Personalisation is the term used for ensuring that every person receiving care and support is able to
set their own personal goals and have choice and control over their type of care and support. It is
important to promote personalisation care as it enables new relationships between service users and
service providers1. It provides a positive shift in power and decision making that enables people to

have a voice, to be heard and be connected to each other and their communities 2. Service users can
control their services, what service they want, what treatment they want and how they want it
delivered. Promoting personalisation care would allow individuals such as Billy and Valerie to
overcome their challenges due to their different needs. Valerie and Billy would have their own team
of multidisciplinary and multi agencies. They would be linked with a specific type of care which suits
them the best resulting in helping overcome their challenges.


In order to get the best personalisation care and support the health and social care service users
will have to be assessed by the local authority who will start by carrying out a care and support
needs assessment to decide whether the service user’s needs meet the national eligibility criteria. If
service users' needs do meet the criteria, the local authority will have to meet these needs.
However, if the user does not meet the criteria then the local authority will enable you to receive
information about other services and ways in which they can find funding in order to meet their
needs. This will allow the service needs to meet therefore empowering them which would improve
their confidence and independence resulting in an improvement in their quality of life 3. For example,
if Billy’s and Valerie’s meet the national eligibility criteria then they would receive personalisation
care for their conditions such as for Valerie she would be supported for hearing impairment, obesity
and mental health conditions. Similarly, Billy will be supported for his visual impairment and asthma.


During assessment procedures the local authority will start by drawing up a care and support plan
which includes discussing with the user how they wish to live their life. For example, if they want
their care to be provided in a residential setting or home. The local authority will look at prevention
services such as aids to make it possible for the person to stay in their own home; for example,
providing information about support available such as help in the home or care in a care home. In
the personalisation care the service user will be involved throughout the whole process and if
necessary an advocate will be arranged. If the local authority is paying for the service user’s care
then the local authority has the right to choose what feels the most effective method. The service
providers will conduct financial assessment to ensure whether the local authority is able to meet the
care costs or if the service user will need to self-fund. If the service +user is funding their own
treatment then they have the rights to make their own decisions about the effectiveness of the
treatment4. If Billy and Valerie fund their own care then they would be given the authority to make
their own decisions. However, if Billy and Valerie meet the criteria due to having either physical or
mental impairment or illness they would be funded by the local authority therefore they have the
right to choose what feels the most appropriate and effective treatment for Billy and Valerie.
