Preliminary Care Coordination Plan

Introduction

The purpose of care coordination is to ensure there is an integration of services that meet

the needs of individuals. Its focus lies in the provision of recovery-focused and collaborative

services to connect people to healthcare services (Hannigan et al., 2018). A primary necessity for

care coordination is the management of chronically sick patients. It creates a challenge of the

illness to personal identity and alteration to social relations and creation of uncertainty. It is even

more worrying since an effect to one sphere of life may affect the other (Hannigan et al., 2018).

For instance, a chronic illness becomes a social problem with its effect on social interaction.

After that, it may result in unemployment and income-related difficulties. Just as it seems

complicated to the patient, so is the impact on service provision calling for family, friends, and

multidisciplinary approaches in care provision (Hannigan et al., 2018). It is for the sole reason

that the interconnected problems of the patient rarely fall into a single profession. Therefore, it

calls for a multidisciplinary approach and a care coordination plan to solve health concerns.

Chronic Obstructive Pulmonary Disease (COPD)

Patients with COPD present with a variety of complex health issues that call for more

than pharmacological interventions. Care coordination helps to solve these complexities and

provide alternatives to quality health care. Through a collaborative approach, it is possible to

meet the patient’s burdens, address not only the respiratory aspect of the disease but also the

systemic effects and comorbidities (Hannigan et al., 2018). The Journal of Chronic Obstructive

Pulmonary Disease states that best practices for care coordination for COPD include pulmonary

rehabilitation, holistic approach, integration of care, and addition of care transition to the care

model (Nici & ZuWallack, 2018). It is only through this that there is a means to meet the
