Unit 5, assignment 2
In this assignment will be discussing the various types of
agencies and how they assist the individuals in the case studies
in this assignment ( Jim & R). I'll also talk about how agencies
and service providers may work together to assist service users
through multidisciplinary teamwork. Then I'll talk about
information management and confidentiality, as well as the
roles and responsibilities of the many multidisciplinary team
members. Finally, I'll discuss multidisciplinary managing
standards of practise and laws, as well as how multi-agency
collaboration may meet service users' needs.
P6: Explain why meeting the needs of the individuals
requires the involvement of different agencies.
Clinical Commissioning Group and why is it important? -
The Health and Social Care Act 2012 established clinical
commissioning groups to coordinate the delivery of NHS
services in England. CCGs are local groupings of general
practises (GPs) that work together to procure the best services
for their patients and the public. They are legal NHS
organisations that are led by clinicians and are in charge of
organizing and commissioning health care services in their
region. There are 106 CCGs in England as of April 1, 2021,
following a series of partnerships. This entails determining local
requirements, prioritising and strategizing, and purchasing
services on behalf of the people from providers such as
hospitals, clinics, community health organisations, and so on.
It's a never-ending process. CCGs must be able to respond and
adapt to changing local conditions on a regular basis. They are
in charge of their entire population's health and are judged on
how well they improve results. CCGs are membership
organisations whose members are local GP practises. Finance,
HR, data management, and contracting are some of the
services provided by commission support units to CCGs. CCGs
,have the option of purchasing services from CSUs or doing
them in-house, as they believe is the most efficient and
suitable. CCGs get CSUs under the NHS England Lead Provider
Framework. (references from unknown author of an article from

Local Authority and why is it important? – Because local
authorities are in charge of public health, CCGs collaborate with
them through health and wellness boards. They collaborate to
design a combined needs assessment and health care plan to
achieve the best potential results for the surrounding area. Any
Local Authority has jurisdiction over the Area in which it
exercises its rights, functions, and authorities in order to carry
out its responsibilities. Local government is in charge of a
variety of essential services for residents and companies in
specific areas. Well-known functions like social care, schools,
housing and planning, and garbage collection are among them,
as are lesser-known duties like licencing, business support,
registrar services, and pest control. Regional and local roads
are the responsibility of local governments to improve and
maintain. In addition, they are in charge of: public parking
services. Officers in charge of traffic. (references from

Health and Well-Being Boards and why is it important? –
they are a statutory forum where political, clinical, professional,
and community leaders from throughout the care and health
system join together to enhance the health and wellbeing of
their community and minimise health inequities. They have
been in place since 2013, and they provide a single point of
consistency in an ever-changing health and care sector. They
are establishing a refocused offer to assist the leadership of
place utilising virtual tools and platforms, for example, owing to
the COVID-19 epidemic, they placed all our efforts on battling
the pandemic. For HWB Chairs and leading members in care,
,health, and wellness, the following recommended leadership
support is offered. Council leaders, clinical commissioning
groups, HWB chairs/co-chairs, HWB vice chairs (CCGs),
Portfolio-holders/lead members with responsibility for adult
social care, and portfolio-holders/lead members with
responsibility for public health under an NHS partnership or
Devo arrangement. ( references from local.gov.uk )

Both service users have various demands that must be satisfied
in order for them to receive the best possible care. Agencies
are organisations tasked with providing appropriate caring and
safeguarding persons, as well as ensuring their independence.
Private organisations, public services which is government
services, and voluntary services (typically charities that benefit
others, such as cancer awareness) are all examples of different
sorts of organizations. The first service user has a stroke that
causes bleeding in his brain. The Stroke Association, a
voluntary organisation, will offer him with assistance. They help
individuals rebuild their lives after a stroke by providing
professional assistance, as well as sponsoring and organising
campaigns to ensure that people who have had a stroke
receive the best possible treatment and support. R will get
information, support, and guidance from the specialists
providing care. The staff at the stroke association understands
that everyone, including him, deserves the finest treatment
possible, so they use a comprehensive process to help that
staff ask the correct questions and deliver the right information
to fulfil his requirements. In addition, he suffers from epileptic
episodes. Epilepsy Action is the organisation that will assist him
with this. Epilepsy Action is a non-profit organisation that offers
support and information to those who have epilepsy. The
agency also informed him that if he or any other individual also
has an epilepsy seizure and is unable to take a job or obtain
funds to pay their bills, they have the right to contact social
security disability benefits and inform them of their
, circumstance, and they will help them find the benefits and
information they require to help them cover their costs and
other fees. NHS England is also another organisation that may
be able to assist him with his ailments. The NHS offers a variety
of programmes that examine various situations in various ways.
R will receive particular care because he had a stroke and has
epilepsy, and they will take care of all of his requirements in a
formal environment. R will be placed in a rehab center by the
specialists on that multidisciplinary team, who will monitor his
progress every 12 hours. They'll also make sure R is in a
comfortable, safe setting. For R's stroke, they will give activities
and community leisure as well as fitness programmes to keep
him active. R will also attend stroke survivor groups where they
may talk about and address any concerns they have, such as
the possibility that it could occur repeatedly and they will be
unprepared. The NHS experts speculate that R's epilepsy may
have been caused by the stroke. The organisation will look after
him and give rehabilitation in order to meet R's wants.
The second service user has kidney failure at stage three.
Reengage is an organisation that might be able to help Jim.
That is a non-profit organisation that creates social contacts for
the elderly when they are at a point in their lives when they are
socially isolated. Jim may be lonesome and socially isolated
now that his wife has passed away and his daughter lives too
far away. Reengage organizations assist older people in
overcoming loneliness by encouraging them to converse and
participate in more interactions. Because the service user is in
a care facility, Jim gets the chance to meet and chat to new
people because he is surrounded by individuals his age.
Social services is another organisation that will assist him. They
will offer emotional and social assistance in addition to
advocacy services. They'll make Jim feel at ease at the care
facility where he'll be residing, and they'll make adjustments to
make him feel protected. Social services give assistance and
cover expenditures in health facilities where Jim may require
