Adrenal Crisis
● Low cortisol - due to Addison’s, exacerbated by stress
● Cortisol
Symptoms
● Collapse, pallor, cold & clammy skin, hypotension, dizziness, vomiting, diarrhoea
Management
● ABCDE, call 999, state “Adrenal Crisis”
● Lie flat, administer O2 15L/min (mask w reservoir bag), give 100mg IM hydrocortisone
Anaphylaxis
Symptoms
● Sudden onset, urticaria, flushing, respiratory distress (wheeze, stridor), hypotension, tachycardia
Management
● ABCDE, call 999, state “Anaphylaxis”
● Lie flat, administer O2 15L/min, give 0.5ml IM adrenaline (1:1000), repeat at 5m intervals until
adequate response. Max 150mg (<6), 300mg (6-12), 500mg (12+)
● IM = in glutes, deltoid, thigh or tongue
Asthma
Symptoms
● SoB, expiratory wheeze. Severe: RR >25min. Life-threatening: RR <8/min
Management
● ABCDE
● Sit upright, 2 puffs salbutamol inhaler (bronchodilator). Can use spacer (improve delivery)
● Unsatisfactory/no response → call 999, O2 15L/min, 1 puff salbutamol every 60s, up
to 10
Cardiac Emergencies
Myocardial Infarction
Symptoms
● Chest pain/discomfort, radiating down arm or to jaw/back/stomach, nausea, SoB
Management
● ABCDE, call 999, state “Heart Attack”
● Comfortable position (upright), 2 x 400mcg GTN spray sublingual, chewed/crushed 300mg aspirin
● Unconscious → start CPR
Known Angina
● Sit down, GTN, wait a few min, repeat GTN. No relief → HA protocol
Choking
Symptoms
● Coughing, struggling to breathe/talk, reaching for throat
Management
● Effective cough → encourage coughing
● Ineffective cough & conscious → 5 back blows, 5 abdominal thrusts
● Ineffective cough & unconscious → call 999, start CPR
