The purpose and the aim of this report is to evaluate and answer the following questions:
P6 --Explain why meeting the needs of the individuals requires the involvement of different agencies
P7 -- Explain the roles and responsibilities of different members of the multi-disciplinary team in meeting the
needs of specific individuals.
P8 -- Explain the arrangements for managing information between professionals
M5--Assess the benefits of multi-disciplinary and multi-agency working for specific individuals with care and
support needs.
M6--Analyse the impact of legislation and codes of practice relating to information management on multi-
disciplinary working.

P6 --Explain why meeting the needs of the individuals requires the involvement of different agencies
When it comes to multi agency team and multi-disciplinary team there is a clear differences. A multi-
agency team involves different bodies, boards and authorities working together whereas a multi -
disciplinary team is a team which plan, enforce, and track an individual's treatment as part of a
team of health and care professionals from various professional backgrounds and positions.
The benefits of having a multi-agency team involved in a individuals care when meeting their
needs is that it delivers a holistic approach as having different professionals that specialise in
different areas of work involved in one persons care means they can meet different needs. Another
benefit would be a decrease in the chances of gaps in their care appearing as there will be multiple
eyes watching out for failures when planning this individuals care. One more benefit would be the
professionals involved in this individuals care can use their skills more effectively and to the fullest
capability.
The importance of having different agencies involved in a individual care is so that they different
needs are looked after, as all individuals have different needs that need to be catered for, making it
difficult for only one healthcare professional to help them with it. For example dietary restrictions ,
When preparing a healthy diet, it's important to take religious and cultural dietary variations into
account. As some individuals may be Muslim and Jewish therefore they must be provided with halal
or kosher diets depending on the religion they practice. The benefit of the multi-agency when
helping with this need is they can access a wider variety of resources as well as cost being reduced.
Another example of a need that a individual may have that would benefit from multi agencies
involvement in their care is self harm, This is the effect of bullying and it can have major impacts on a
individual growth and development especially their emotional development. It can lead to a
individual feeling unsafe , alone ,isolated and a decrease in self confidence. The benefit of multi
agencies when helping with this need is the fact that they will be a wide variety of resources
available to help aid this individual with there issues with self-harm and the multi agency support
will be coordinated for the individuals needs.
In reference to case study one service users Valerie has a lot of needs for example she was just
admitted to surgery following surgery on her appendix that just burst , she has mental health
problems due to the loss of her parents ,she is currently being bullied by local teens around her area
due to her weight ,she is also vegetarian and has hearing impairments that have not been access or
given support for. This is a lot for one healthcare professional to deal with as there is a lot of needs
to under take and they may not posses the skills and knowledge on how to deal with all Valerie
different needs. Therefore having a multi agencies involved with her care and providing her with
support will benefit her more as her care will be carefully planned out and it will deliver a holistic
approach as different professionals will meet different needs on top of all of that it will mean she
will be provided with a wider range of resources and help as the different professionals will have
access to a wider range of resources. The different type of agencies that might be involved in Valarie
care is the NHS, Clinical Commissioning group ,mental health foundations etc.
In reference to the second case study service user Tremayne has also got alot of needs for example
he is autistic , he has recently been admitted to a larger city hospital due to the fact he had fallen
and broken his leg in three places , he has a wheat allergy and doesn’t understand it that well and
his struggles to control his temper which results in outburst leaving the other service users around
him distressed. When dealing with a service user like Tremayne who has behaviour problems it can
