P6: Explain why meeting the needs of the individuals requires the involvement of
different agencies.

To meet the needs of the individual's care, Multi agency teams (MAT) P6 need to work
together as well as multi disciplinary teams (MDT) which is a team of professionals in
the health and social care sector working together to create the best and efficient care
for service users. Multi agency teams refers to the two different professionals from
different services and organisations sharing their information on a service user to
provide holistic care P6and that safeguarding and confidentiality is met. (2. Health
executive- 2016)

For the first case study of Brenda would require the use of the multi agency teams
which help Brenda to promote independence. The use of the sheltered accommodation
manager would help provide support to Brenda as it would allow her to receive help and
support while allowing her to keep up with cleanliness as her mental health may not
allow this sometimes, so by providing this accommodation for Brenda will help meet her
needs. P6 (3. housingllin- 2016) Moreover, the use of MDT in Brenda's care would be
the asthma nurse specialist, psychiatrist and the G.P to name a few. These would all
collaborate together to give Brenda the best care in order to empower her and improve
her overall wellbeing by providing strategies and techniques to overcome her asthma
and mental health issues. The use of the G.P would be able to refer Brenda to a
specialist in asthma as well as a psychiatrist as well as her overall health which helps
provide specialised care for Brenda and improve her mental health while keeping
confidentiality as the GP could tell the psychiatrist. P6 (1. class notes) On the other
hand, the use of the MAT regarding Mrs Montanelli and Maria would be the night sitter
which supports Maria as it lets her get sleep as well as become less stressed and
improve her overall health. The use of the MDT would be the specialist dementia nurse
and the bilingual community care assistant would help Mrs Montanelli communicate and
will advise her on ways to support herself in making adjustments as well as Maria in
knowing what happens in the future. (4. Anon- 2019).

Organisations have to work together to provide holistic and tailored care to Brenda, Mrs
Montanelli and Maria. The first organisation is the Clinical Commissioning Groups
(CCG) P6 which were established as part of the Health and Social Care Act in 2012.
CCGs commission any local community service provider that meets the NHS standards
such as local hospitals and health centres. The aim for these are to create better holistic
care for patients and provide a range of services from mental health services to urgent
care services. (5. NHS-2019) The first case study is Brenda and she requires a GP
(which make up the CCG) who would refer Brenda to a community psychiatric nurse for
her mental health (which are commissioned by CCG) to talk through problems and
provide practical advice and support. Moreover, Maria and Mrs Montanelli would need

, the use of CCG`s aswell such as the use of G.P to oversee her overall health and the
bilingual community care assistant which would be able to translate English to Italian for
Mrs Montanelli so she will be able to communicate with other professionals in her care,
so that she gets tailored care. Furthermore, the CCG boards are responsible for the
budget of the NHS and are subject to the freedom of information act. The second
organisation that has to work together is the local authorities (LA`s). LA`s commission
publicly funded social services such as service users own homes and residential care
services. Their aim is to achieve holistic and personalised care P6 in the community
for all service users who need the care. The LA's take guidance from the Adult Social
care Framework in order to be up to standard and provide the best possible care.
Linking this to Brenda she would use the local authorities such as the sheltered
accommodation to provide Brenda with the opportunity to go into the accommodation
which enables other healthcare professionals to come into and help her as well as have
suitable adaptations for her. (1.class notes) Moreover Maria and Mrs Montanelli will also
require the use of the local authorities such as the local social worker who has been
asked to do an assessment of the home and to see if Mrs Montanelli should be put into
a care home as well as providing support for Maria in the care of her mother.

To meet the needs of service users such as Brenda, Maria and Mrs Montanelli local
authorities and CCG work with the MDT and MAT to provide tailored care which
enables them to live their best possible life. They do this by commissioning these
services to achieve community based support to promote health and wellbeing by using
the resources provided such as skills and resources like multiple professionals in each
sector to provide a range of services for the service users. As previously said they work
with local authorities such as housing for people like Brenda this would have enabled
her to move into sheltered accommodation and help her live independently as
possible. P6 The manager and the MDT working with Brenda would have discussed
the right care plan and assessed her needs so she is best suited in going to sheltered
accommodation so that adaptations and the best care can be given as Brenda isn't
capable of living alone. Furthermore, this would also benefit Maria and Mrs Montanelli
as these organisations follow frameworks such as Making it Real statements which
would support Mrs Montanelli getting personalised care that will meet the legal duties
and achieve a more productive and positive relationship with the carers so that no
discrimination would take place and that Mrs Montanellis opinions are heard. (6.SCIE-
2018) This would allow Mrs Montanelli to feel empowered by the organisations
providing good quality care.

Health and wellbeing boards P6were established under the health and social care act
2012 which states that the local health and care system can work together to improve
the local population. They promote greater integration and partnership of the NHS and
public health by bringing together the local authorities to carry out a joint assessment
with the CCG`s to create a health and wellbeing strategy for the local population. The
boards are chaired by a senior local authority who has been elected by the population
but a part of the board must be a representative of the relevant CCG and other
commissioning groups. The boards aim to integrate public services for health and social
care by assessing the needs of the local population, encouraging integration by more
