the decade that preceded 1997, specific government-supported safety initiatives were set up, such

standards that could assist identify safety failures, and incident reporting varied. NHS trusts

lacked reporting and staff training systems. In 2000, the proof was published in an independent

government report (An Organisation with a Memory). It found that adverse occasions that would

cause harm to patients occurred in 10 percent of admissions to NHS hospitals (about 850,000

adverse reactions each year). Meanwhile, there were around 6,610 adverse incidents related to

medical devices each year, including 400 deaths or serious injuries.
