P6 – Explain why meeting the needs of the individuals requires the involvement of different
agencies.

Multi-agency working is about different services, agencies and teams of professionals and other
practitioners working together to provide services that meet the needs of different individuals. For
example, social services and health providers work together to ensure community safety. They
collaborate with doctors and nurses to protect patients and ensure they are treated with
kindness and care and given the best possible treatment at all times during their care plan. This
might involve informing the different agencies about any difficulties a patient is having following
medical advice.

Why agencies work together?

Many issues would occur if they failed to work together as a service user’s life could be put at risk as
a result of them not making multi-disciplinary teams aware of any care plans. Through working
together, the system is more efficient and cost effective as each service provider is a specialist in
their own area of care so they are able to share information and resources rather than spending
time and money on finding resources themselves. This way service providers can ensure that all
areas of care are covered and it enables them to work together in order to plan and provide the best
service possible for users.

Multi agency working is important because a lack of co-ordination between services can create
problems:

- Service users may need to deal with numerous agencies whilst looking after themselves
- They may need to repeat the same story to different agencies
- Services may be forced onto people when they do not need them
- They can be inefficient and therefore not cost effective
- Services users have a range of different needs that require different expertise

- The CCG was established to help the general community achieve great medical outcomes by
analysing the needs and requirements of the community, determining objectives, and purchasing
resources on behalf of the public from health and service providers for example in order to provide
service users with the best care.

CCGs link to Martin’s case study as the government may have had to put extra money and resources
in for people like Martin because he struggles with both his mental and physical health. His negative
mental health makes his physical health worsen for example his skin condition. It will not get better
on its own which is why extra resources are required as it may result in skin cancer. Martin visits his
GP for regular check-ups to ensure that his skin condition is under control meaning it will get better
and not worse. The GP may undertake some tests to make sure everything is how it should be. This
includes the CCGs as they are there to help minimise the challenges that Martin may be dealing with
and they also fund the facilities that he may require to ensure he is well looked after and being given
the support he requires.

The CCGs also benefit Nusrat as she may seek urgent medical help as a result of her epilepsy
therefore the clinical commissions group would provide the funds so an ambulance could be sent
out in order to help Nusrat recover safely and because of them they may save her life. They provide
better and faster care as a result of their quick communication between service users, providers and
other health and social care workers.

Furthermore, Alice would benefit from the CCGs as they can aid her in trying to enforce a better
more healthy lifestyle, one where she is not drinking as much. The CCGs would make Alice aware of
the situation she is putting herself and the damage she is causing to her body in order for her to
realise the change that needs to be made and the benefits of having a healthier lifestyle. This will
therefore lead her to be a lot happier and healthy both physically and mentally.

National Eligibility Criteria:
What is the National Eligibility Criteria (Care Act 2014?)

The national eligibility criteria is a single framework for determining whether any assessed care and
support needs meet the requirements for support from the Local Authority. The Care Act 2014 is the
law that sets out how adult social care in England should be provided. It requires local authorities to
make sure that people who live in their areas: receive services that prevent their care needs from
becoming more severe.

Eligibility Criteria:

They must be provided to all people who appear to need care and support, regardless of their
finances. They must also be carried out with involvement from the adult and their carer or someone
