Cycle of Planning

managed for a service user are called cycles of planning. An individual's
preferences and needs should be reflected in this record. Health and social care

There are several steps involved in a planning cycle:
The first thing an assessor must do is gather all the information they need about
a service user before performing an assessment. They provide care and
treatment for and on behalf of other organizations and services that support the
individual.
After completing the first phase of the planning process, the second step
involves identifying and assessing the needs of the service user to whom care,
or treatment will be provided. Individuals should be able to access the best
possible treatment options based on their interests and needs, so they can be
treated in a way that meets their best interests. Likewise, monitoring the
treatment and care of a service user should continue once the treatment has been
implemented to ensure that their needs are being met.
It will be necessary to monitor and modify the treatment plan if needed after it
has been implemented. In order to ensure that a treatment plan is effective and
continues to work for the service user, it will need to be evaluated and reviewed.

Secondary care - Specialist services are usually involved in secondary care. In
order to receive specialized care and support, individuals often seek out these
services and professionals. A medical doctor in a specialist hospital clinic, a
specialist doctor, a physiotherapist, and a dietitian are examples of professionals
who may provide secondary care. Referrals are often the first step in receiving
secondary care.
Tertiary care - Often provided to seriously ill individuals, tertiary care is
highly specialized. Patients in these conditions require specialized hospital care,
such as intensive care in an intensive care unit.
