professionals to collaborate to plan and deliver care and support to the individuals with
whom they work, resulting in better outcomes for service users and higher quality care
being delivered to individuals. Health care professionals who provide care to individuals are
required by law to keep records about your health and any treatment or care users received
from any NHS organisation. These records aid in providing the best possible healthcare to
patients. Under the Data Protection Act of 1998, multi-agencies and multidisciplinary teams
must ensure that patients' personal confidential data is handled in transparent and

In health and social care, there is never total confidentiality. This is because breaching
confidentiality can sometimes be in the best interests of the client or another party whose

and social care system is asked to contact him and notify him of their findings. Tremayne
would lose faith in his service providers as a result of this, and he would feel mocked and as
if his problems were insignificant.

Billy would be reassured to know that his confidentiality is being protected because he is
wary of strangers and has no one to rely on since being kicked out of his own house by his
mother's boyfriend and living on the streets, Billy had to fend for himself and had no help,
so knowing his confidentiality is protected would put him at ease. Billy will also feel more
confident that he is being well cared for if he knows his social worker values his opinions
and respects his privacy and choices. This will boost his self-esteem and make him more
likely to share personal information that will help identify his individual needs and improve
his care plan. Billy's needs would not have been met if the social worker had not followed
the correct code of practice and failed to maintain professionalism and confidentiality.
First, by failing to maintain confidentiality, Billy puts himself at risk of discrimination and is
more likely to develop low self-esteem as a result of his fear of discrimination and
judgement. Furthermore, the social worker will be unable to effectively meet Billy's
individual needs because the positive, trusting bond they had has now been broken due to
the social worker's failure to maintain confidentiality. As a result, Billy is less likely to tell his
social worker about his concerns for fear of having his information spread, reducing the
effectiveness of care.
Sharing information among practitioners and organisations is critical to provide the right
care to the right individual at the right time. Concerns about sharing information should
not be allowed to obstruct the need to protect and promote the welfare of adults, children,
and young people who are at danger of abuse or neglect. Proper information exchange is a
