- =All controlled medications
-Keep in refrigerator - =Augmentin, Cefpodoxime, Cefuroxime, Cephalexin,
Penicillin. Latanoprost (xalatan) too.
-May cause drowsiness, care when operating machinery - =Antipsychotics,
analgesics, some antidepressants (mirtzapine, trazodone), dopamine agonists,
antihistamines, muscle relaxants, some anti-hypertensives with CNS effects,
anticonvulsants, all controlled depressent/narcotic drugs, hypnotics.
-May cause blurred vision - =Amiodarone, anticholinergics, isoniazid,
isotretinoin, PDE5's (sildenafil), tamoxifen, voriconazole
-Do not take with alcohol - =Disulfuram, metronidazole, opiods, nitrates,
tramodol, BZDs, hypnotics, anticonvulsants, antipsychotics, some
antidepressants, muscle relaxants, insulin, metformin, sulfonylureas
-Avoid prolonged sun exposure - =Sulfa antibiotics, tetracyclines, isotretinoin,
some NSAIDs, metronidazole, isoniazid, diuretics
-Take on empty stomach - =Ampicillin, Atripla (efavirenz specifically),
bisphosphonates, iron, PPIs (always before eating), levothyroxine, opana,
bactrim, voriconazole, transplant drugs (mycophenolate/tacrolimus)
-Take with food - =Coreg (both forms), metformin (both forms), mevacor,
niacins, nsaids, opiods except Opana, steroids
-Take with water - =Bactrim, Bisphosphonates.
-Avoid grapefruit juice - =Simvastatin, atorvastatin, amiodarone, CBZ,
verapamil
-Avoid in prego - =ACE/ARBs, CBZ, lithium, NSAIDs, phenytoin, topiramate,
VPA, methotrexate, statins, warfarin
-Discoloration of urine, sweat - =Levodopa, metronidazole, nitrofuranoin
, -Peanut/soy allergy - =Combivent (ipratropium/albuterol)
-Seperate from dairy products/antacids - =Tetracyclines, quinalones
-Do not refridgerate - =Metronidazole, Bactrim, Biaxin, Acyclovir
-Glass bottles required - =Nitroglycerin, Amiodarone, Cyclosporine, Paclitaxel
-Protect from light - =Levothyroxine, epinephrine, fentanyl, versed,
amiodarone, doxycycline, octreotide, hydrocortistone/methylprednisolone
-D5/W only - =Amiodarone, Amphotericin B, azitrhomycin, nitroprusside,
Bactrim, mycophenolat,e neupohen
-NS only - =Phenytoin, Erythromycin, Levothyroxine, Metronidazole,
Daptomycin
-Acyclovir warnings - =do not refrigerate, short stability
-Bactrim warnings - =Do not refrigerate, D5W only, short stability. Monitor
potassium, renal fxn
-Metronidazole warnings - =Do not refrigerate, NS only
-Amiodarone - =Glass bottle only, (PVC sorption if hung for >2 hrs) protect
from light, D5W only. Requires filter. Patients need to be in hospital for
initiation. No grapefruit juice. May cause blurred vision.
-Fentanyl - =Protect from light
-Doxycycline - =Protect from light, short stability
-Insulin - =No PVC (sorption, drug concentration goes down)
-Azithromycin - =D5W only
-Linezolid - =Protect from light, do not shake if a suspension.
-Furosemide - =Light sensitive, store at room temperature.
