What is a care pathway?
According to Age UK care pathways are designed for those with a condition or that have
certain needs. They are a way of setting out the process of the best practise to be followed
in the treatment and care of a client or patient. These should also be evidence based.

Purpose?
From this you can interpret that the purpose of a care pathway is to ensure that the best
care possible is given to the patient. This provides support and the appropriate action to be
taken to fulfil this.
What is: Primary, secondary, and tertiary care?
Primary care is within the community, an example of this is the GP.
Secondary care is within the hospital, for example a consultant.
Tertiary care is specialist clinics for example a plastic surgeon.
What are formal carers?
As suggested by Beryl Stretch, formal carers include those with a profession, this includes
doctors, nurses, and outside organisations. These can be hired by the individual or by an

What are informal carers?
Informal carers are those without a profession, for example friends and family of the patient.
Involvement of other agencies and organisations:
Sometimes, when producing a care pathway, it is important to include other agencies, for
example meals on wheels; private care agencies, for example care homes; and charities –
these are non-profitable organisations, e.g. Diabetes UK.
Care strategies:
Care strategies are long term approaches to care for the client/patient. This may be
monitoring of their condition and medication and possible changes needed to their care
pathway.


I will be producing two case studies of 2 patients with different physiological disorders and
providing them with a care pathway.
