What is person centered care?
Person centered care is placing each individual at the centre of their care
and support. It must fit the individual, rather than the individual being
made to fit existing routines or ways of doing things, it is tailored to them
as individuals.
The person centered values tell you how to work in a person centered
way. The care values are central work in health and social care. They are
principles that guide workers to understand right from wrong and are
about what is important when caring and supporting individuals.
Person centered care is tailored to the individual. It has been proven to
improve health outcomes, whilst also enabling them to live a fulfilling life.
It gives staff the knowledge, skills and confidence for them to work
together in order to provide appropriate care. And overall it improves both
care quality and patient experience.
The 6c’s also come into person centered care. So, the care giver must
include the use of care, compassion, competence, communication,
courage and commitment whilst they are doing their job as a health
professional.
