P6: Explain why meeting the needs of individuals require the involvement of different agencies

This assignment tends to focus on explaining why meeting the needs of individuals require the
involvement of different agencies.

Clinical commissioning groups in England were created following the Health and Social Care Act in 2012
(What were clinical commissioning groups? 2023). This service offers responsibility for the planning and
commissioning of health care services for their local area. They also assess local needs, decide priorities
and strategies, and buy services on behalf of the population from providers such as hospitals, clinics, etc.
As well as this, CCGS are responsible for commissioning healthcare including mental health services,
urgent and emergency care, and community care. The professionals involved are an elected governing
body made up of GPs, other clinicians including a nurse and a secondary care consultant. This agency
can meet Valarie's needs by assessing that her obesity is because there are a lot of cheap takeaway
shops in her area, which can also be seen as a local need. One strategy they can come up with to meet
Valarie's needs is the public gym park, as she would be able to work out and it would be for free. As well
as this, many other people would be able to join as it could be seen as a local need. Another way the
agency can meet Valarie's needs is by setting up Valarie in a mental health service which would involve
counselling, psychological therapies, and support services. This would help Valarie as she would be able
to heal from her trauma of losing her parents in a professional manner and able to receive techniques
and advice to help her cope. On the other hand, this agency can meet Tremaynes' needs by assessing
how in Treymane's local area, there aren't many glutens free shops. The agency can then open gluten
free take-away shops which could help Tremayne as he would stop getting ill from his wheat allergy, and
it would help those who eat gluten free foods in the area. Another way the agency can meet Tremaynes'
needs is by setting him up for counselling. This is because Treymane's autism makes him have frequent
temper outbursts. The therapy will teach Tremayne the technique to remain calm in heated situations,
making more people likely to interact with him.

Common Assessment Framework (What is the Common Assessment Framework?: Explained 2023) is a
process used to identify children’s unmet needs and support them. Using the CAF, an assessor works
with the child and their parents or carers to understand the issues at hand and then formulates an
action plan to provide the best support possible. This service offers to help the carer support their child.
It can lead to a quick solution or help to identify extra support if needed. The agency ensures that
everyone involved with the child works together to support them. A skilled practitioner who has been
trained to complete the assessment is involved in the CAF. However, any practitioner (teachers, support
staff) can complete a pre-assessment checklist if they think a child may be at risk, even if they haven’t
been trained to do a common assessment themselves. The agency could've helped Valarie when she
was younger with her mental health, with a Counseller. The agency wouldn’t be able to help her now as
(The Common Assessment Framework for children and young people) The CAF is generally used with
children and young people up to the age of 18 This is because Valarie lost her parents at a young age,
which is a great loss. The CAF would be able to support Valarie with a Counseller who can help Valarie
manage her new emotions and allow her to speak about the loss. The agency could have helped
Tremaynes when he was younger with his autism, with a therapist. Due to his autism, Tremayne often
has temper disruptions. The therapist can allow Treymane to talk about his feelings and what triggers

him into having a temper disruption as well as guide him and help him manage his anger. Treymane
being able to manage his anger and talk about his feelings is beneficial for Tremayne as he would be
able to maintain healthy relationships and people would want to socialize with him, whereas before
people might not have wanted too due to his anger outburst and lack of communication.

National eligibility criteria (The National Eligibility Criteria 2023q

The Education Health & Care Plan (Service, Children with special educational needs and disabilities
(send) 2014) is a legal document which describes a child or a young age person (up to 25) special
educational needs, the support they need, and the outcomes they would like to achieve. An EHC plan
can give a child or young person extra educational support. It can also give parents and young people
more choice about which school or other setting the child or young person can attend. EHC plans
identify educational, health and social needs and set out the additional support to meet those needs. An
individual can request an assessment themselves if they’re aged 16 to 25. The professionals involved are
the school or setting, specialist teacher (for children and young people with hearing and/or visual
impairments), a speech and language therapists and /or other health professionals, an Educational
Psychologist, and a Social Worker. This agency can meet Valarie's needs as she is 25, and if she is willing
to go into education or work, they would be able to identify her needs and support her. They can do this
by allowing her to be taught with a specialist teacher due to her hearing aids as well as a mental health
professor who can help her if she's ever upset in lessons. This would be able to help her as she can hear
well in lessons and her feelings are managed, increasing her determination to continue in her work or
education. This agency could have met Tremaynes' needs when he was younger by identifying his needs
and supporting him. They can do this by having a therapist with him so that if he was ever to have an
anger outburst in the class, the therapist can help him calm down. This would be able to help Tremayne
when he was younger, as having the therapist would prevent him from having these anger outbursts,
meaning he can take on more knowledge from his class and not disrupt class for other people as well.

P7 Explain the roles and responsibilities of different members of the multidisciplinary team

M5 Assess the benefits of multidisciplinary team and multi-agency working for specific individual with
care and support needs

D4 Evaluate how multi agency and multidisciplinary working can meet the care and support needs of
specific individuals

A multidisciplinary team (MDT) is a group of health and care staff who are members of different
organizations and professions (e.g. GPs, social workers, nurses), that work together to make decisions
regarding the treatment of individual patients and service users. MDTs are used in both health and care
settings.

One professional who could be involved in Valarie's MDT is a Counseller. Counsellors work with
individuals who are experiencing emotional and psychological problems to help them bring about
effective change and/or enhance their wellbeing. A counsellor's responsibility is to make people feel
relaxed, to be patient, empathetic and non-judgmental. Counsellors hold sessions with individuals and
groups in a safe and confidential environment. Some counsellors specialize in working with problems,
