of different agencies.
Individuals have various needs and, in some cases, have more than one. It is important to
meet these needs and understand that these needs can only be met through the use of
having multiple specialists working together with one individual providing their expertise in
the role of caring for the individual and ensuring that they are able to rehabilitate
effectively. People have different needs for example an individual who suffers from Diabetes
will have to see specialists from the endocrinology department and will also have dieticians
to ensure that they are eating the correct foods including a balanced diet which is important
to a Diabetic as it could trigger their blood sugar level to rise or fall which can have a strong
impact on the overall well-being of the patient. The patients can suffer from vision
problems, weakness and fatigue and even face on a more serious scale amputations.
Multi- disciplinary teams communicate about the patient they are working with through a
care plan in which the needs of the patients are stated and also how the needs are to be
met. The care plan will state the patient’s wants and preferences. Having multiple agencies
in the involvement of the care of specific individuals will allow a more holistic approach of
care to take place where all their needs are met at once. Each need is seen to by a specialist
who focuses his entire skill set on that particular need. This allows the service being
provided to be enhanced and the patient can recover better and have a better chance of
rehabilitation. Brenda can be someone to look at as Brenda has many needs that need to
be cared for and this requires the use many professionals to achieve this. Brenda is a severe
asthmatic and will require regular consultations with the pulmonologist who will regularly
tests her overall fitness and peak flow to ensure that she is able to maintain the condition
and make sure it does not get worse. The condition must stay controlled as it may cause
asthma attacks and not seeking help can result in something fatal. Brenda may suffer
asthmatic attacks hence if she is taken into hospital, the staff that are receiving her need to
know about her condition as they need to be prepared to perform any treatment and also in
emergency situations to perform life-saving treatment. Brenda will also have regular
consultations with her GP who will make referrals for her to relevant professionals and also
prescribe her any medications that she is required to have.
