Scenario Title: Spinal Cord Injury with Respiratory Failure

Brief Description of Case: A 46- year-old female pedestrian struck by a truck is admitted to ICU with multiple
injuries, including a thoracic spinal cord injury, a right flail chest and
pneumothorax, and a liver laceration. While awaiting OR for spinal stabilization, she
decompensates into mixed respiratory failure and hemodynamic instability. She will
require ventilatory support, escalation of vasopressors, and will develop a tension
pneumothorax post-intubation.

Goals and Objectives
Educational Goal: Manage respiratory failure in a patient with Spinal Cord Injury.
Objectives: 1. Manage mixed respiratory failure in a trauma patient with multiple injuries.
(Medical and CRM) 2. Identify and manage neurogenic shock in a trauma patient.
3. Consider a differential for hypoxia and shock post-intubation.
4. Optimize resource utilization during medical crises.
5. Resuscitate a patient in a challenging, unfamiliar environment.
6. Seek help from consultants when faced with a deteriorating patient
requiring definitive management.
