- Seborrhee:
o Acne ontstaat thv grote talgfollikels in seborroïsche driehoek (
≠ aandoening van talgklier zelf) wel aandoening van
talgklierproductie
- Retentiehyperkeratose:
o keratinisatie ↑ vastzittende hoornlamellen microcomedo
gesloten comedo (whitehead) open comedo (blackhead,
melanine)
- Bacteriele colonisatie en ontsteking:
o ↑ anaerobe bacterien: cutibacterium acnes (C. acnes)
o Lipasen: zetten triglyceriden om tot vrije vetzuren
(irriterend, comedogeen)
o Proteasen: afbraak proteinen: ontsteking
o Onsteking + vrije vetzuren trekken chemotactische factoren
aan: ↑ witte bloedcellen: geeft rode, ontstoken witte puisten
het is dus multifactorieel

5.2.3. Acne cosmetica
A) Niet-inflammatoire folliculaire respons: toename vorming witte
(gesloten) comedonen
B) Inflammatoire folliculaire respons (folliculitis): irritatie tast het
follikelepithelium aan en veroorzaak papels en pustels
- Comedogene substanties
o Lanoline
o Esters van vetzuren (vb. isopropylpalmitaat)
o Minerale en bepaalde plantaardige en dierlijke olien
o Bepaaalde rode pigmenten in bv blush
o Bepaalde emulgatoren bv sorbitanoleaat
sterk afhankelijk van concentratie, zuiverheid, galenische vorm,
penetratie in haarfollikels, aanwezigheid van irriterende stoffen
(vb NaLS) uittesten van afzonderlijke ingredienten én afgewerkte
producten
5.2.4. Iatrogene acne
- Geneesmiddelen die acne kunnen veroorzaken of verergeren
o Androgenen
o Progestagenen intra-uterien, implantaat (spiraaltje zoals flo
had)
o Steroiden
o Andere: halogenen, lithium, vit B12, tuberculostatica, anti-
epileptica, corticosteroiden

- Mechanische wrijving of manipulatie, occlusie
- Straling (zon, kobalt, rontgen, … )

o Matige tot ernstige acne die niet beantwoordt aan de klassieke

- Tretinoïne
- Aanbrengen op volledige zone + volhouden want effect te verachten
na 4 tot 8 weken
- Nevenwerking: irritatie
- Niet tijdens zwangerschap!
- Kan uitdrogend zijn dus hydraterende creme erbij aanraden,
druppels voor droge ogen kan ook, lippenbalsem
