Probleem bij wegraking is dat je het niet ziet gebeuren als dokter. In verleden
wegrakingen. Lastig uit te vinden wat het echt is. Vaak onderzoek neer epilepsie en
hartziekte en dan is het het niet, heel duur. Patiënt vaak amnetisch moet het van
ooggetuigen hebben. Trekkingen/geen trekkingen. Kunt van alle stoornissen in de
hersenen bewusteloos raken.

 Traumatisch: hersenschudding
 Niet-traumatisch

o Gegeneraliseerde epilepsie
 Tonisch, Clonisch, tonisch-clonisch

= wegraking door cerebrale hypofusie
1. Reflexsynxope – verkeerde beslissing. Autonome ZS zet circulatie uit
2. OH – onvoldoende druk, faalt om druk omhoog te houden
3. Cardiaal – pomp stuk
Sympathische zenuwstelsel houdt arteriolen minder of meer dicht (vasoconstrictie).

2 soorten verschijnselen/symptomen:
1. Hypoperfusie hersenen (& retina), kunt alleen zeggen het was een zyncope en
geen epilepsie bv.
 Schokken, bewusteloosheid, zwart zien. Voor flauwvallen werd het zwart
voor de ogen. Eerst ogen zwart omdat je nog oogbal druk hebt die het
moeilijker maakt om bloed doorheen te persen.
2. Gekoppeld aan oorzaak
 Reflexsyncope: bv. misselijk, bleek, zweten
 OH: bv. pijn schouders
 Cardiaal: bv. hartkloppingen

Cardiaal – ritmestoornis
Bv. ventrikel tachycardie
 Uitlokking:
o Geen, kan ook liggend (bij uitlokkingen ofwel ritmestoornis of wel
epilepsie, rest heeft wel uitlokkingen)
o (schrik, wekker, koud water: erfelijk verlengd QT-syndroom)
 mechanisme
o pomp werkt niet
 hartkloppingen, bekende hartziekte: bij meeste mensen is het al bekend dat ze
ziekte aan het hart hebben. Hartklopping aangeven en daarna flauwvallen.

Cardiaal – structurele hartziekte
Bv. klepziekte
 Uitlokking:
o Geen, of tijdens lichamelijke inspanning
 Mechanisme
o discrepantie vraag & aanbod
 hebben al ziekt van het hart en kunnen ook pijn op de borst bij krijgen.

Orthostatische hypotensie
= bloeddruk daalt tijdens staan
 uitlokking:
o opstaan. Duidelijk evrhaal, uit voorzorg gaan zitten
o Na inspanning, bij stoppen met inspanning. Autonome zenuwstelsel krijgt
vaatjes niet meer dicht.
 mechanisme
o onvoldoende vasoconstrictie door falen sympathische ZS
o bloed verzamelt in venen
o bloedruk daalt bij staan/ stop inspanning
