A: Theme 1 : Eating disorders
Diagnostic criteria and hallmarks features of Anorexia Nervosa and Bulimia
Nervosa and Binge-eating Disorder.


Eating disorders are becoming common place in Southern Africa. Eating is a normal
basic requirement for survival. Most people take it for granted that eating is a daily
activity that is enjoyed and is part of life. But for some it has become an obsession. As a
result, eating has become a disorder. There are three types of disorders that will be
discussed. And in order for a person be diagnosed with an eating disorder they must
meet the criteria as laid out in the DSM-5 and the ICD-10. (Jordaan,2014)


Anorexia Nervosa clinical Picture: “Means loss of appetite”, it is distinguished by
three core criteria, first is behavior aimed at achieving significant weight loss. Second is
a great fear of becoming obese and finally a disturbance in their own perception of body
size or shape.(APA,2000,Halmi,2000)
The DSM-5 considers Anorexia Nervosa to be a disorder. But for a person to fit this
diagnosis they must fit certain criteria. A. The person restricts energy and food intake
relative for the requirements for normal healthy living. Which leads to low body weight in
context to age, sex, developmental trajectory, and physical health. Significantly low
weight is defined as weight that is less than the minimally normal for children and
adolescents. For a more definite diagnosis to determine what is normal. Doctors and
Dieticians look at the Quetelet’s body mass index. If the body weight is maintained at
least 15% below the BMI or 17.5 % or less. Then the patient can be described as
Anorexic. For a complete diagnosis of a mental disorder. B. The client must show an
intense fear of gaining weight or becoming fat. And there must be persistent behavior
that prevents weight gain, even when the client is at a significantly low weight already.C.
If there is a mental disturbance in the way, the clients body weight or shaped is
experienced. Also if there is a lack of recognition of the current low weight. If the dread
of fatness persists as an intrusive, over –valued idea as stated in the ICD-10. The
international Classification of Diseases (ICD-10) criteria explains that there must also be
a widespread endocrine disorder involving the hypothalamic-pituitary axis in women as
amenorrhoea and in men as a loss of sexual interest and potency. There is also
elevated levels of growth hormone, raised levels of cortisol, changes in the peripheral
metabolism of the thyroid hormone and abnormalities of insulin secretion. Girls will also
experience a delay in pubertal events such as breast not developing. (APA,2013) and
(ICD,WHO,Geneva,2007).
The subtypes of Anorexia Nervosa are, restricting type to Binge- eating/Purging type.
Restricting types means a person will restrict food intake or engage in excessive
exercise to lose weight. Binge –eating/purge type is a subtype that is characterized by
